0126147139 2016-06-08 Preliminary Restoration No Violations This inspection was conducted as a preliminary restoration status check. The access road has been reclaimed and crops have been planted. The new lease road can be accessed from Kirby Rd. off of State Route 13. Inspection meets ORC 1509.072 requirements. Identification was observed at the tank battery. No violations. 1125621259 2016-04-19 Production Wells Violations Noted Inspect well location as a status check. The well was equipped with a plunger lift system. A chart was observed at the wellhead. The area around the well has not been reclaimed yet. The preliminary restoration is due 6/3/2016. My inspection revealed that the air pit has not been closed. It was observed that pit water has breached pit wall. A water hole approximately 10 feet x 10 feet was observed just outside the pit. A low range Quantab titrator for Chlorides was used to field test water outside of the pit in two different places on opposite sides of the water hole. The test resulted in a 7.0 on the Quantab scale which equals approximately 447 ppm for chlorides on one side of the water hole and a 2.6 which equals approximately 69 ppm for chlorides on the other side of the water hole. The water in the pit was field tested as well. The pit water resulted in a 8.8 on the Quantab scale which equals >644 ppm for chlorides. According to ORC 1509.072 (A) all pits containing brine and other waste shall be filled within two months of completion. The company will be advised to close pit as soon as possible. Preliminary restoration is not complete. The tank battery consists of (1) 210 barrel tank inside an earthen dike and (1) horizontal separator. Identification was not observed at the well or tank. The company previously was asked if they could try to mitigate the odor from the tank battery. They have yet to get back with me regarding the odor. 1848997112 2015-11-20 Casing String Witnessed No Violations I witnessed the cementing of the 8.63 casing on Kilbarger rig #5 for Artex Oil Company. Heath Halstead was on location for Artex. Formation Cementing was contracted to cement casing. Formation broke circulation and good cement returns were observed. Cement did not drop. A basket was not used. The cement ticket was received. Time of inspection: 7:30 AM.
1867216860 2016-03-03 Production Wells No Violations Inspect tank battery in regards to a phone call from adjacent landowner regarding an odor from tank. The tank battery consists of (1) 210 barrel tank inside an earthen dike and a horizontal separator outside dike. A phone call was placed to Joe Snider with Artex and it was suggested that a charcoal filter be installed. This well was drilled in a non-urban area and therefore a charcoal filter is not required.
