1096553594 2013-12-09 Production Wells No Violations This inspection was conducted as a standard production well status check. The well was equipped with a pumping unit and motor. A GPS reading was taken at the wellhead. It was not pumping during inspection. The polish rod indicates signs of recent production. The well produces into a tank battery that consists of (2) 100 barrel tanks and horizontal separator. This is a shared tank battery with p# 3864. A GPS reading was taken at the tank battery. The inspection revealed that brine is being drained on the ground near the tank battery. A low range Quantab titrator was inserted into a pool of water approximately 15 feet behind the tank battery. The Quantab titrator results showed >600 ppm for Chlorides. An identification sticker was posted at the tank battery and labeled Bradley 4 with an address of 15135 Brushy Fork Rd. Newark, Oh 43056. RBDMS records show the last year for reported production was in 1995. The well owner will be contacted and is required to do the following: register well and place under their bond, send missing production reports to Central Office, post identification at wellhead, dispose of brine through a registered brine hauler. Note: Photos taken at tank battery location for documentation purposes.
1509728186 2017-11-03 Exempt Well Pre Transfer No Violations This inspection was conducted as an exempt well pre-transfer inspection. The well was equipped with a pumping unit and motor. It was capable of production. The well produces into a tank battery that consists of (2) 100 barrel tanks and horizontal separator. This is a shared tank battery with API permit number 3864. A 100 barrel unloading tank was also observed at these coordinates: N 40.02802 W -82.26795. An identification sticker was posted at the both tank battery locations and labeled Bradley 4 with an address of 15135 Brushy Fork Rd. Newark, Oh 43056. Time of inspection: 11:45 AM.
