Sediment fence is still surrounding the location. Access road is stone. Well is not connected to equipment or the main line. Work on the main line is being done now. Crews are working to connect LEDCO 1 & 4 to Gather Cos line.
0590124922 2007-12-14 Drill / Deepen / Reopen No Violations Surface cement job. Spud date 12/13/2007. 16.25 hole concentrically drilled for 16 conductor, set 145 of 16. Drilled 12.375 hole for the 9.625 TD on hole 274. Ran 253.35 of 9.625. The casing is new 26lb. Drill rig is Cross Rock Rig #4. Pikeville Ky. I was called out at 11pm on 12/13/07. Cement job did not start until 3am. Good circulation and cement returns to the surface. There was a spike of pressure at the end of the cement job that caused cement to be blown out of the hole. Cement truck had extra cement and mixed some to grout to the surface. Cement stayed at the surface. BOP looks to be in good shape. Rubber diaphragm looks new. BOP test in 8 hours.
0991647917 2008-02-28 Production Wells No Violations Well is not connected to the main line. The line is being installed at this time. The silt fence is still up and the location looks good. The id and fencing will be installed after the well is connected to the main line.
0997080179 2007-12-14 Drill / Deepen / Reopen No Violations BOP test. Cement sample is hard. 2 full ABC Fire extinguishers are located in the dog house. The flow line was not secure. It was being held down by one small trac hoe. I told them to stake it off and suggested they use full salt water storage tanks to chain off the flow line. Jeff Price (NYTIS) said that he can have 2-500Bbl tanks on location. BOP failed first test due to dirt around the flange. This was repaired and cleaned. Second test passed held 500psi for 15 minutes. They bled the pressure off through the flow line.
1226687129 2008-08-28 Administrative Inspection No Violations Dave Finley called and asked permission to install a mechanical separator and a brine tank on the LEDCO #4. I instructed Dave on the spacing requirements and the urban fencing requirements. Dave asked if they could install the separator outside of the fence temporarily and see if the well makes enough fluid to have the equipment be permanent. I said that would be fine as long as the fencing is extended as soon as it is determined that the equipment is permanent.

1843923394 2013-04-02 UrbanProduction Wells No Violations Jon Scott took me to this well site and other LEDCO lease wells to inspect good examples of urban township production wells, urban well identification signs, fence construction, etc. I took pictures of signs and fence to use as good examples for compliance notices on future urban well inspections. Besides the gas wellhead in the fenced enclosure, there was one 2000-gallon poly tank used for brine storage, and a low profile liquid/gas separator greater than fifty feet from the wellhead.
