0965691416 2017-06-12 Orphan No Violations My inspection found the well is located in a brick vault with a steel lid. The vault is over 2 ft deep and access to obtain measurments of the well casing was not possible. The well has a line plumbed into the casing and both valves are currently closed. The well is situated in the main drive of the cemetary. There are no overhead obstructions.
1299401428 2018-02-20 Final Restoration No Violations I inspected this well location as an evaluation for final restoration following plugging and abandonment that was completed on 7/28/17. There had not been any existing production equipment present at the time that the well was discovered. The existing vault was left intact with a well identification plate affixed to the casing top. At the time of this inspection, the vault interior was partially filled with water but the casing top was dry, with no indications of leakage or gas detected. No other activity is needed and final restoration is complete.
1308606977 2017-07-28 Plug / Plug Back No Violations I was on location to inspect and observe preliminary activity being conducted as part of plugging and abandonment being executed by RA Gibson. The well was discovered by cemetery personnel and is comprised of a 6-inch diameter casing, with C-clamp head and associated 1-inch supply line. It is suspected that at one time, this well and the #6-0507 well, located nearby were connected to a common supply line. The wellhead is located in a circular subsurface masonry vault with a cast iron lid. There was detectable gas odor in the vault.. The residual pressure was blown down and the casing lid was removed. A 3-inch diameter bailing device was lowered into the well bore and residual water was bailed from the well bore. A continuous plug consisting of 9-sack grout was top-loaded without problem or incident.
1521141157 2017-07-31 Plug / Plug Back No Violations I inspected this well location as a follow-up to plugging and abandonment which was completed on 07/28/17. At the time of this inspection, the well plug was stable with no signs of gassing or problems. The existing well vault is to be left intact per the discretion of the landowner (City of Painesville) and may include a passive vent, to be established later.
