1313080352 2016-03-08 Orphan No Violations The Cleveland Electric Illuminating company contacted the Division of Oil and Gas in 1996, Case # 96-001, to report a possible orphan well on their property was leaking gas. The Perry Nuclear plant is located on this property, south of the well. Inspector Kohl responded to this 1996 complaint and found the well located approximately 75 south of Lake Erie, in the side of a steep bank. The well is approximately 20 down gradient from the top of the bank. The 1996 inspection report in the Orphan Well file stated that fifteen feet of 4 1/2 steel casing was exposed above ground level and gas was venting from inside the casing. My inspection today showed the slope of the bank where the well is located appears to have slipped further north towards Lake Erie since 1996. The area is overgrown with phragmite grass and the well is not visible from the top of the slope. The same utility pole shown in the 1996 picture laying near the well, is still near the well. The well casing is sheared off at the slopes ground level and due to the steep grade the well is inaccessible without a rope. A gas odor is evident in the area of the well. The well is approximately 41 yards north of an electric pole and approximately 30 yards northeast of an electric pole. The slope around the well will need an engineers review to determine if it is possible to stabilize the slope enough to access and plug the well. The area south of the well is a stable flat grassed area with room for moving in a rig and equipment placement. Three sets of cement barricades and chain link fences block access on the lane to the well from Lockwood Rd, which is located to the east and also from entering the area around the plant from the west. Part of the access road, east of the nuclear plant, has been removed. Note: Leonard Meeks is the current contact at the Perry Nuclear plant to gain access to the well. His extension is 5515, this number is entered at a key pad inside the entrance

Todays inspection was conducted with Leonard Meeks with First Energy, Supervisor-G. Chini, and Inspectors-

1481211050 2016-12-07 Orphan No Violations Todays inspection was conducted with Inspector Perry and Josh Schiering to attempt to determine the depth of this well. We were accompanied by Timothy Zak and Leonard Meeks with First Energy. A fourth set of cement barriers have been added to the drive back to the well. The well is equipped with 4.5 inch O.D. steel casing and is open at the top of the casing. Inspector Perry lowered the line on his depth finder into the open casing and ran out of measuring line at 883. An approximate 12 foot piece of 4.5 inch O.D. steel casing is laying on the ground next to the well. The ground where this well is located appears to have slipped during the life of the well leaving the well in the side of a steep slope approximately 20 feet from the top of the slope. Adding the 12 feet of casing to the 883 feet (measured before running out of line) makes the well over 895 feet deep. Inspector Perry measured water at approximately 680 feet from the present ground level where the well is located. A gas odor is evident near the well. Further measuring line will be added and the depth measured at a future date. The Division will not be able to access this area until Summer of 2017 due to restrictions placed by First Energy. Access to the well will possibly be from the east side of the Nuclear Plant.
