0672898871 2017-06-12 Orphan No Violations My inspection was conducted today to update the status of Orphan Case Numbers, 1990 12 158 and 159, original complaint number 90-075, initiated by the Lake County Metro Parks in 1990. Two non productive gas wells are located in the woods near the eastern property line of Lakeshore Reservation Metro Park. Lake Metro Park has no history or information for these wells. There are no known records for these two wells and this well was assigned permit # 6-0227 for tracking purposes. Todays inspection found a 62 inch by 62 inch cement block vault without a cover. The orange, plastic safety fence that was around the vault is now laying on the ground on three sides of the vault. It is 36 inches from the top of the vault to the top of the well. The well cap extends approximately two inches from the bottom of the vault. Debris and dirt partially cover the well and regulator. Additional debris has accumulated in the vault since the last inspection and had to be removed from around the well to expose the casing. The well is equipped with a seven inch outside diameter steel casing with a three-bolt cap. A twelve inch diameter pancake regulator is connected to a port on the side of the well casing with a 1.65 inch steel line. A valve is located between the well and the regulator. A one inch steel line is evident near the regulator. There is an occasional gas odor. My Sperian Gas Meter detected 3% LEL, O ppm H2S. There are no evident tanks or other producing equipment in the vicinity of this well. This may be a Shale well. The Ohio Fuel Gas map does not show any wells in this area. Note: This well is very near the property line, a survey may be necessary to confirm the well is on Lake Metro Park property. Lake Erie is approximately 450 feet to the north and the well is east and approximately 36 feet uphill from a creek that leads to Lake Erie. This area is heavily wooded and not easily accessed from the Metro Park property due to the topography and creek. The well would be easie

The well would be easiest accessed from the adjoining property to the east, which is currently owned by Roy and Sherrie Blalock. The first well is northeast
