At the time of this inspection, I observed the following: a wellhead with plunger-lift system and associated tank battery located on a rural property. The shared-use lease road was dry and in good condition. Fittings and connections were secure. Portions of the tank battery and wellhead enclosure were broken down and/or missing. There was no identification signage. I contacted the well owner and was awaiting a response at the time of this report filing.

1361879786 2013-02-15 UrbanProduction Wells No Violations I visited the unit pursuant to a reported complaint that the operator of the unit was dumping produced brine into the ravine located adjacent to the unit. On my arrival, I observed the producing unit to consist of the following: a wellhead with mechanical plunger lift enclosed within a 6 board on board enclosure. The enclosure gate was not attached. The associated tank battery located 50 feet north of the wellhead, consists of 1 100 barrel (BBL) tank, 1 50 BBL tank, mechanical separator, and associated appurtenances. Production safety equipment required by 1501:9-9-05 is present. I noted that the tank battery secondary containment drain was open. I tested an area of ponded water within the secondary containment for the presence of chlorides. The test indicated a maximum chloride level of 200 parts per million (ppm). In addition, I inspected the ravine area located west of the producing unit. The ravine drains intermittently to lower lying areas to the north. At the time of my insepction, there were several areas of ponded water, but no continuous flow. I tested several areas of ponded water along the course the ravine, both upgradient and downgradient of the unit. None of the samples indicated chloride levels that exceeded 28 ppm. I did not see any evidence of improper discharge of wastes. I contacted Jesse Mulnar of Autumn Ridge regarding the missing gates and open dike drain.

The well and battery are located in separate enclosures one the side of a private access road which is shared with 2 residences. Fittings and connections at the wellhead and battery are secure and free from signs of leakage or other problems. Process safety equipment is present. The load-out is plugged and secure but needs a lock. There is no identification or signage at the well or the battery. Evaluation of drainage and other surficial features is limited due to snow cover. I did not observe any evidence of impact relating to on-going production activity.
2045067435 2018-06-07 Exempt Well Pre Transfer No Violations I inspected this well as part of proposed transfer to exempted domestic status. At the time of this inspection, I observed the following: a wellhead with mechanical plunger lift system and associated tank battery in separate, board-on-board enclosures. The well had an active domestic connection to the house at 10500 Painesville-Wareen Road, which was listed as Lake County Parcel No. 08A0110000350 . The well is on the same parcel, based on Lake County Auditor geospatial map data. The production fittings were present but closed. There remained a sales line connection that was inactive. The chart record was inactive. The measured distance from the well to the nearest occupied house, not on the parcel, was 297 feet. There was no other neighboring, occupied structures within 200 feet. I advised the well owner that the commercial sales line connection would need to be disconnected and removed prior to transfer to domestic well status.
