Process safety equipment (spark arrestor, carbon filter, and low-pressure relief valve) are present. Gates are locked. No problems noted.

The well and associated battery are located in an open portion of an otherwise wooded lot along Crile Rd. The well and battery are accessed via a short, unpaved road that enters from Crile Road. At the time of this inspection, I observed the following: a wellhead with mechanical plunger lift system in a board-on-board enclosure. Fittings and connections at the wellhead were secure. The 8.625-inch casing is valved at surface but not gauged. The battery is comprised of two 100-barrel steel production tanks and separator in a separate board-on-board enclosure. Enclosure gates were locked and secure. Hatches were closed and sealed. Secondary containment was present and intact. Fittings and connections were secure and free from signs of leakage or problems. I walked the areas surround both the well and battery and did not observe any signs of impact relative to the production activity. Signage was present. I noted the following safety issues: The load-line valves were not locked. The owner must remedy the following by 04/08/2016: secure the load-line valves with locks. Place and maintain a gauge at the surface casing nipple as required by permit condition. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.

At the time of the previous inspection, I had noted that the load-line valves were not secure with locks as required and the surface casing annulus was not gauged. At the time of this inspection, I observed the following: the access road to the well and battery, which is comprised of a gravel-paved road that enters from Crile Road and travels 700 feet to the well and battery, was in good condition with no drainage problems or wash-outs. The separate enclosure gates were locked and secure. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure and free from leakage. I walked the areas surrounding both the well and the battery and found no signs problems or impact to the surrounding in-situ features. I did observe the following maintenance and safety deficiencies; the load line valves remain unsecured, the ID signage had not been updated to reflect the recent change in ownership, and the wellhead enclosure has been partially dismantled, and the surface casing annulus is not gauged. I contacted John Magnussen with David Oil and advised him about the continuing deficiency. He indicated that the items are being worked on as they are able. I contacted Richard Osborne Sr. with David Oil Co., and as of the date of this report, I have not received a response.
1344173238 2019-02-14 UrbanProduction Wells No Violations I conducted an compliance notice follow-up inspection for compliance notice 873314079, regarding the load-line valves nneding locks installed and the 8.625-inch casing was not gauged nor fitted with a pressure relief device as required under the special permit conditions which were part of the drilling permit. At the time of this inspection, it was observed that the plunger lift assembly equipped well was not in production and the surface casing nipple was not gauged or fitted with a pressure relief device. The tank battery consisted of two 100 BBL tanks and a mechanical separator. The tank valves were still not equipped with locks and the identification still needed updated to reflect current owner information. Lease access to the site was stable with a gate at the entrance.

At the time of this inspection, the noted deficiencies remained. In addition, the well signage remains unchanged since the change in ownership that occurred in 2016. Repeated calls to the owner have not produced any demonstrable action or results.

I have contacted the company on several occasions since the issuance of the compliance notice. The company has reported that problems with personnel and/or lack of funds, have prevented them from being in compliance.
2005622329 2015-02-13 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well is located in a commercial/light industrial area on the north side of Crile Rd. The well is comprised of a wellhead with mechanical plunger lift located 200 feet from the tank battery. The battery, mechanical separator, and battery are in separate board-on-board enclosures. The enclosure for the battery is unlocked and I found the gate to be left open. Load outs at the battery are bull-plugged. Connections and fittings are secure and free from signs of leakage or problems. At the time of this inspection, there are 10 to 15 inches of snowfall, which hinders a thorough inspection of surficial features (e.g. access road, secondary containment interior. I notified Tim Reilly (John D. Oil & Gas) about the need for a lock on the gate. I will follow-up in 30 days.
