At the time of the previous inspection, I had noted the following: the load-line valves were not secure with locks, and the tanks were missing carbon filter and spark arrestor. At the time of this inspection, the noted deficiencies remain. In addition, the signage has not been updated to reflect the change of ownership that ocurred in 12/2016. I contacted John Magnussen with David Oil who indicated that the issues are being handled as they are able. I inspected the wellhead and surrounding area and found no signs of problems otherwise.

At the time of this inspection, I found the following: Fittings and connections at the wellhead were secure and free from leakage. The flow-line connection from the wellhead had been disconnected and the gate valves at the well were closed. The tank battery was comprised of a 210-barrel steel production tank with separator unit located in the back portion of a larger rural parcel. The following conditions remained extant: Load-lines remained unsecure. Safety equipment which had been noted to be lacking previously (e.g. spark arrestor, low-pressure relief valve, and carbon filter) remained so. The surface casing was not plumbed to surface and gauged as required. The owner has indicated that the deficiencies will be fixed but will not provide a timetable.

At the time of this inspection, the noted deficiency items remained. In addition, the well signage remains unchanged since the change in ownership that occurred in 2016. Repeated calls to the owner have not produced any demonstrable action or results.

At the time of this inspection, I observed the following elements: a wellhead with pump jack system in a board-on-board enclosure. A tank battery comprised of one 210-barrel steel production tank, one 100-barrel steel brine tank, a mechanical separator. Fittings and connections were bull-plugged but not locked. Low-pressure relief valve, spark arrestor, and carbon filter were not present. Fittings and connections at the wellhead were secure and free from signs of leakage or problems. The 8.625-inch casing is not exposed and gauged as required by specific permit condition. The owner must remedy the following by 04/04/2016: secure the load-line valves with locks. Fit the production tank with spark arrestor, low-pressure relief valve, and carbon filter as required. Expose and property gauge the 8.625-inch surface casing annulus as required by permit condition. The battery is comprised of 1 210-barrel steel production tank and 1 100-barrel brine tank, and separator unit. The wellhead and battery are both in an isolated portion of the property and not enclosed. The load-outs are bull-plugged. The ID and secondary containment are good. No problems noted.

In addition, the unit signage had not been updated since the change of owner that had occurred in 2016 and there was not fencing or enclosure at the wellhead or battery. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.
