At the time of this inspection, I observed the following: the load-line valves were locked but the 210-barrel tank remains in place, but was not being actively produced into. I had previously contacted the company on several occasions since the issuance of the compliance notice regarding the deficiencies. The company had made pledges to remedy the deficiencies
0314137541 2015-04-22 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The wellhead is located behind the referenced house on Ravenna Road. The wellhead and battery are located in a mostly wooded area with small clearings for the wellhead and battery. The well is comprised of a mechanical plunger-lift system in a chain-link enclosure. Fittings and connections at the wellhead are secure. The battery is comprised of one 210-barrel steel production tank and one 100-barrel brine tank and separator unit in a separate board-on-board enclosure. Fittings are secure. Load-outs are locked. The containment dike is intact and dry. I walked the areas outside of the wellhead and battery including areas down gradient topographically and did not observe any signs of impact or drainage problems associated with the unit. The access road is unpaved by well drained and passable.

The wellhead includes a mechanical plunger lift located in a 6-foot wooden enclosure. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. I did not observe any leaks, spills, or other signs of contamination. No violations noted.

At the time of this inspection, the well was equipped with a plunger lift assembly and not in production. The well was fenced and had a locked man door. The tank battery was comprised of one 210 BBL tank, one 100 BBL tank and a mechanical separator. The 100 BBL tank has not been removed or disconnected from the other production equipment. The tank valves and pickup lines were plugged and locked. The diked tank battery was fenced with a locked man door. Legible identification was present on location and the lease access was stable.

The tank battery was comprised of one 210-barrel production tank, one 100-barrel brine tank, and separator in a board-on-board enclosure. Gates were locked and secure. Secondary containment was intact and dry. Tank lids were closed and sealed. Production reports were current. The load line valves were plugged and secure. I observed the following safety deficiency: The surface casing nipple was exposed but not gauged as required under a special permit condition. I contacted Oasir, Inc. and advised them about the need for a gauge at the surface casing.

At the time of this inspection, I observed the following additional deficiency: the 100-barrel steel stock tank was observed to have serious and substantial corrosion of the sidewalls resulting in failure of the tank integrity. The tank must be taken out of service immediately, and removed from the production system within 30 days.
