The wellhead is located at the terminus of an unpaved residential road with sparse, isolated development within a wooded area. The wellhead and battery are located in a clearing in the woods past the house. The well is comprised of a mechanical plunger-lift system in a board-on-board enclosure. Fittings and connections at the wellhead are secure. The battery is comprised of one 210-barrel steel production tank and one 100-barrel brine tank and separator unit in a separate board-on-board enclosure. Fittings are secure. Load-outs are locked. The containment dike is intact with some ponded rain water. I walked the areas outside of the wellhead and battery including areas down gradient topographically and did not observe any signs of impact or drainage problems associated with the unit. The access road is unpaved by well drained and passable.
0713521846 2017-09-20 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: The load lines were noted to be unlocked and unsecure. At the time of this inspection, the indicated deficiencies had been fixed. I inspected the remaining portions of the system and found the fittings and connections to be secure and free from leakage or problems. I walked the areas surrounding the well and battery and found no signs or indications of impacts or problems.

The well and battery are located on a larger rural parcel, in a wooded clearing. Access to the wellhead is via a dedicated access road which was in good condition. Fittings and connections were secure and free from leakage. Fittings and connections were secure. Secondary containment was intact. The load-lines were plugged and secure. The well was in active production. The charter record was active and current. I walked and inspected the areas surrounding the well and did not observe any signs of problems.
0929051858 2012-11-07 Production Wells No Violations The unit is comprised of a 2-tank battery (1 210-barrel production tank and 1 100-barrel brine tank) and mechanical separator in an 8-foot wooden enclosure. The wellhead includes a mechanical plunger lift located in a 6-foot wooden enclosure. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. I did not observe any leaks, spills, or other signs of contamination. No violations noted.
1327345506 2012-01-10 UrbanPreliminary Restoration Violations Noted Meet land owner during inspection. Land owner is concerned about drainage accumulating on the north side of the lease road and causing trees to fall. Water accumulates in a low area on the land owners property. Low area could be connected to a drainage ditch that is west of the low area. The land owner is also concerned about a seep that has formed in the area between the wellhead and the tank battery. Sent Tim Reilly an e-mail notifying him of the landowner concerns. Land owner is happy with the condition of the site accept for the drainage. HB-278 equipment needs installed on the tank battery. Called Jim from John D informed him that the well needs HB-278 equipment installed on the battery, charcoal filter, spark arrestor, sealing hatches and low-pressure relief valves.

Secondary containment, spacing, and signage is good. Process safety equipment (spark arrestor, carbon filter, and low-pressure relief valve) are present. Gates are locked. A drainage culvert appears to have been added along the lease road. No current problems with surface water drainage are noted.

At the time of the previous inspection, I had noted that the load-line valves were not secure with locks as required. At the time of this inspection, I observed the following: the access road to the well and battery, which is comprised of a gravel-paved road that enters from Ravenna Road and travels 1,200 feet to the well and battery, was in good condition with no drainage problems or wash-outs. The separate enclosure gates were locked and secure. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure and free from leakage. I walked the areas surrounding both the well and the battery and found no signs problems or impact to the surrounding in-situ features. I did observe the following maintenance deficiency; the ID signage had not been updated to reflect the recent change in ownership. I contacted the well owner on

At the time of this inspection I observed the following: The well and battery were located in a largely wooded area behind a cluster of newer residential development. The partially shared access road was dry and passable. The wellhead mechanical plunger lift system was located in a secured and locked enclosure. The tank battery was comprised of one 210-barrel production tank, one 100-barrel brine tank, and separator in a board-on-board enclosure. Gates were locked and secure. Secondary containment was intact with some standing storm water which was free of signs of free-phase oil or other indicators of leakage. Tank lids were closed and sealed. I noted the following maintenance/safety deficiencies: The load line valves were plugged but not secured with locks, the surface casing nipple was not exposed and not gauged as required under a special permit condition. The owner must do the following by 05/15/2016: secure the load-line valves with locks, properly gauge the surface casing nipple per special permit condition.
