1235175970 2014-04-18 UrbanProduction Wells No Violations I inspected the producing unit in response to a complaint I received from a local resident. The complainant was concerned about some open pits she had observed recently near the tank battery. The findings of my inspection are as follows: The producing unit is comprised of a wellhead with mechanical plunger lift and associated tank battery located in close proximity. The wellhead and battery are in separate chain-link enclosures which are locked. The battery consists of 210-barrel steel production tank and 50 barrel brine tank, with separator in a third, separate chain-link enclosure. Unit spacing meets requirements, and ID placard and secondary containment is present at the battery. I did not observe any signs or indications of leaks, spills, or other problems. There are several large, shallow excavations near the area of the well and battery. The excavations or pits, appear to be related to on-going activities at the property which include handling and storage of mulch and stone. The pits do not appear related to the well production. I advised the complainant that the pits do not appear to be a concern with regards to oil and gas production issues.

1456933751 2016-03-02 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. The well and associated battery are located in a secure, gated facility utilized for materials storage. At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger lift system in a chain-link enclosure, a tank battery comprised of one 210-barrel steel tank, one 100-barrel steel tank, a mechanical separator in separate chain-link enclosures. Fittings and connections were bull-plugged but not locked. Low-pressure relief valve, spark arrestor, and carbon filter were not present. At the time of this inspection I observed both storage tanks to be severely corroded and failing in mechanical integrity. The owner must remedy the following by 04/01/2016: remove the failing tanks from service, secure the load-line valves with locks. Place ID signage at the entry gate. Fit the new production tank with spark arrestor, low-pressure relief valve, and carbon filter as required. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.

At the time of the previous inspection and issuance of the compliance notice, I had noted the following: Fittings and connections were bull-plugged but not locked. At the time of this inspection, the indicated issues of non-compliance remain. In addition, the ID signage reflects the previous owner and has not been updated since transfer in 12/16. I contacted John Magnussen with Rockefeller Oil who indicated that the issues will be addressed as they are able.
