0593821926 2017-08-01 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with pump jack system and associated battery in a small clearing at the back of a residential parcel. Fittings and connections at the well and battery were secure. I noted that the access road had become overgrown with vegetation. In addition, I noted the wellhead board-on-board enclosure was partially broken down. I notified the well owner about the need for maintenance.

1184113123 2019-03-04 UrbanProduction Wells No Violations I conducted an inspection on this well. The well was not in production, at the time of this inspection. The valve on the tubing and the valve on the flow line were closed. The pumping unit was no longer attached to the wellhead. The rods were still present in the tubing. The diked tank battery was comprised of two 100 BBL tanks, one 50 BBL tank and a mechanical separator. Valves on the separator were open, however the chart on the separator showed dates of 5-7-18 to 5-14-18. Legible identification was present at the tank battery. The lease access to the well and tank battery was stable and a locked gate was installed at the lease access entrance. I contacted Petro Evaluation Serv. Inc., main office and spoke with office personnel inquiring on the status of the well. The person whom I spoke with stated that she would contact Steve Franks the field supervisor, regarding my message.
1373988147 2013-07-16 UrbanProduction Wells No Violations The producing unit consists of a wellhead with pumpjack unit and an aboveground storage tank (AST) battery with one steel 2 140-barrel production tanks, one poly 50-barrel brine tank, mechanical separator, and metering run. The unit ID is present and meets requirements. The battery secondary containment is good. The tank battery is not enclosed. Oil is being collected into a 50-barrel poly tank located adjacent to the wellhead enclosure. There is no secondary containment. I contacted Steve Franks of Petro Evaluation Services. He indicated that the tank is being used for fresh water treatment for salting problem.
1535803390 2009-10-30 Preliminary Restoration No Violations Preliminary restoration meets the requirements of 1509.072 ORC.
