0225772274 2016-02-18 UrbanProduction Wells Violations Noted I inspected this producing well as part of routine inspection activity. At the time of my inspection, I observed the following elements: a wellhead with mechanical plunger lift and tank battery consisting of two 100-barrel steel, low-boy storage tanks, with mechanical separator. The battery and wellhead were in in separate board-on-board enclosures. Fittings and connections are free from leakage. The production tank had a spark arrestor, a low-pressure relief valve, and a carbon filter as required. The dedicated access road is in good condition. The enclosure gates were not locked and or secure. The load-line valves were not locked and must be secured with locks. The well ID sign is missing. The owner must remedy the following by 03/25/2016: secure the load-line valves and enclosure gates with locks. Supply a compliant ID sign. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.

The company has reported that problems with personnel and/or lack of funds, have prevented them from being in compliance.
1397990517 2015-02-06 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. The well is located within the confines the Lakeland Community College campus. The wellhead and battery are located at the northeastern-most fringe of the campus and is accessed via a dedicated access road from the main loop drive. My inspection noted the following: The well is comprised of a wellhead mechanical plunger lift and tank battery with 1 100-barrel production tank and 1 100-barrel brine tank with separator unit in separate board-on-board enclosures. At the time of my inspection, the area is covered with up to 14 inches of snow and, as such, elements of the system are obscured with snow cover. Connections and fittings are secure and free from signs of leakage or problems. Load-outs are plugged. Gates to the enclosures are locked. Evaluation of battery interior, lease road condition, and other surficial features is not possible due to snow cover.

At the time of the previous inspection, I had noted that the load-line valves were not secure with locks as required. In addition, I had noted that the unit ID was missing. The battery enclosure gates were locked and secure, however, the wellhead gate was unlocked and open. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure and free from leakage. I walked the areas surrounding both the well and the battery and found no signs problems or impact to the surrounding in-situ features.
