0075393147 2016-02-24 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following elements: a wellhead with gas-fired pump jack system and tank battery with one 210-barrel production tank, and one 100-barrel, steel brine storage tank. . Process safety and air emission control devices (low-pressure relief valve, spark arrestor, and carbon filter) were present as required. I walked the areas around the wellhead and battery and did not observe any signs of impact relating to the on-going production activity. Access to the well and battery was afforded by a short, dedicated, unpaved access road which was in good condition. I observed the following deficiencies: the load-line were not secured with locks. The owner must remedy the following by 03/25/2016: secure the load-line valves with locks. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.

In addition, the well signage remains unchanged since the change in ownership that occurred in 2016. Repeated calls to the owner have not produced any demonstrable action or results.

I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.
