0133318925 2017-09-20 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: The load lines and well enclosure gates were noted to be unlocked and unsecure. At the time of this inspection, the indicated deficiencies remained. In addition, the ID signage remained unchanged and had not been updated to reflect the change in ownership that had occurred in 2016. Repeated calls to the owner have not produced any demonstrable action or results.
0287613372 2016-02-22 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger lift system and tank battery with two 100-barrel, steel production tanks. The well and battery were located in separate board-on-board enclosures. Process safety and air emission control devices (low-pressure relief valve, spark arrestor, and carbon filter) were present. I walked the areas around the wellhead and battery and did not observe any signs of impact relating to the on-going production activity. Access to the well and battery was afforded by a short, dedicated, unpaved access road. Portions of the road were flooded and may indicate active management of surface drainage is warranted. I observed the following deficiencies: the wellhead and battery enclosure, and load-line valves were not secured with locks. The owner must remedy the following by 03/25/2016: secure the load-line valves with locks, secure the enclosures with locks. The wellhead includes a mechanical plunger lift located in a 6-foot wooden enclosure. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. I did not observe any leaks, spills, or other signs of contamination. No violations noted.

I contacted Jim from John D and notified him that HB-278 safety equipment needs installed on the well (charcoal filters, vent stacks with low-pressure relief valves, hatch lids with functioning seals and lighting arrestors).

1474791660 2015-04-22 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The wellhead is located at the terminus of an unpaved residential road with sparse, isolated development within a wooded area. The wellhead and battery are located in a clearing in the woods near the road terminus. The well is comprised of a mechanical plunger-lift system in a board-on-board enclosure. Fittings and connections at the wellhead are secure. The battery is comprised of one 100-barrel steel production tank and one 100-barrel brine tank and separator unit in a separate board-on-board enclosure. Fittings are secure. Load-outs are locked. The containment dike is intact and dry. I walked the areas outside of the wellhead and battery including areas down gradient topographically and did not observe any signs of impact or drainage problems associated with the unit. The access road is unpaved by well drained and passable.

In addition, the ID signage has not been updated to reflect the change of ownership that occurred over 12 months prior to this inspection. I have contacted the company on several occasions since the issuance of the compliance notice. The company has reported that problems with personnel and/or lack of funds, have prevented them from being in compliance.
