0356244041 2017-09-01 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: the load-lines and well enclosure were no secured with locks and the system was missing spark arrestor, low-pressure relief valve, and carbon filter. In addition, the identifying signage had remained unchanged since the previous change of ownership and had not been updated to reflect new ownership, and the wellhead enclosure was partially missing. At the time of this inspection, the aforementioned deficiencies had not been remedied. The owner has made repeated pledges to address the issues, but with no demonstrable progress.
1119341009 2017-04-14 UrbanProduction Wells No Violations I inspected this producing well as a follow-up to a previously-issued Compliance Notice. At the time of the previous inspection, I had noted that the load-line valves were not secure with locks as required. At the time of this inspection, I observed the following: the access road to the well and battery, which is comprised of a dedicated gravel-paved road that enters from Garfield Road and travels 2,100 feet to the well and battery, was in fair condition with some potholing but was passable. The battery enclosure gates were unlocked and unsecure. The wellhead enclosure was partially dismantled. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure and free from leakage. I walked the areas surrounding both the well and the battery and found no signs problems or impact to the surrounding in-situ features. I did observe the following maintenance and safety deficiencies; the load line valves remain unsecured with locks, the ID signage has not been updated to reflect the change in ownership. The wellhead enclosure needs replaced. In addition, the tank battery is missing the spark arrestor, low-pressure relief valve, and carbon filter as required. I attempted to contact both company representatives John Magnussen and Richard Osborne and, as of the filing of this report, no representative of the company has responded.

1316790764 2016-02-18 UrbanProduction Wells Violations Noted I inspected this producing well as part of routine inspection activity. At the time of my inspection, I observed the following elements: a wellhead with mechanical plunger lift and tank battery consisting of one 210-barrel steel production and one 100-barrel steel brine storage tank, with mechanical separator. The battery and wellhead were in in separate board-on-board enclosures. Fittings and connections are free from leakage. The production tank did not have a spark arrestor, a low-pressure relief valve, or carbon filter as required. The dedicated access road is in good condition. The enclosure gates were not locked and or secure. The load-line valves were not locked and must be secured with locks. The owner must remedy the following by 04/01/2016: secure the load-line valves and enclosure gates with locks. At the time of this inspection, I observed the following: there were no locks at the load-line valves or enclosure gates. In addition, the ID signage remains not updated since the transfer of ownership of the company over 12 months ago. The wellhead enclosure had been partially dismantled. I have contacted the company on several occasions since the issuance of the compliance notice. The company has reported that problems with personnel and/or lack of funds, have prevented them from being in compliance.

2005472845 2014-05-23 UrbanPreliminary Restoration No Violations I inspected this producing unit to evaluate conditions for preliminary restoration. The well was completed in 08/15/2007. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger lift system. The wellhead is located in a closed and locked board-on-board enclosure. The 8.625-inch casing is valved to surface but is not gauged. There is currently 250 PSI showing at the production tubing and 500 PSI showing at the 4.5-inch casing. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located southeast and adjacent to the wellhead. The battery is comprised of 1 steel 210-barrel production tank, 1 100-barrel steel brine tank, and a horizontal separator which is located in a third board-on-board enclsoure. The battery is located in a separate board-on-board enclsoure which is gated and locked. The load-outs are bull-plugged. The secondary containment at the battery is present but very low and not adequate to contain a release. There is ID signage at the battery which is legible. The area formerly occupied by drilling equipment, including former pits, has been restored and reclaimed. There is well established grassy vegetation throughout the area. The area of the well is mostly level, but sits on the edge of a drainage with high relief to the south. There is an intermittent stream at the bottom of the hill adjacent to the battery. Surface draininage is towards the intermittent stream. I did not observe any signs of gully formation or washouts. The extant lease road consists of a dedicated, gravel-paved road with entrance from Garfield Road, and travels east and then south for approximately 1,800 feet from the main road.. The lease road is currently in good condition and free of washout or erosional deficits. Beyond the area cleared for the wellhead and tank battery, the areas surrounding the unit are heavily wooded, with no neighboring homes or structures within eyesight. I notf
