In addition, the identifying signage had remained unchanged since the previous change of ownership and had not been updated to reflect new ownership. At the time of this inspection, the aforementioned deficiencies had not been remedied. The owner has made repeated pledges to address the issues, but with no demonstrable progress.

At the time of this inspection, I observed the following: there were no locks at the load-line valves or indicated process safety equipment. The ID signage had been updated. I have contacted the company on several occasions since the issuance of the compliance notice. The company has reported that problems with personnel and/or lack of funds, have prevented them from being in compliance.
1492176364 2017-04-14 UrbanProduction Wells No Violations I inspected this producing well as a follow-up to a previously-issued Compliance Notice. At the time of the previous inspection, I had noted that the load-line valves were not secure with locks as required. In addition, I had noted that the tanks were lacking the required process safety equipment (spark arrestor, low-pressure relief valve, carbon filter). At the time of this inspection, I observed the following: the access road to the well and battery, which is comprised of a gravel-paved road that enters from Norton Parkway and travels 300 feet to the well and battery, was in good condition with no drainage problems or wash-outs. The separate enclosure gates were locked and secure. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure and free from leakage. I walked the areas surrounding both the well and the battery and found no signs problems or impact to the surrounding in-situ features. I did observe the following maintenance and safety deficiencies; the load line valves remain unsecured with locks, the ID signage had not been updated to reflect the recent change in ownership, and the process safety equipment remains missing. I attempted to contact both company representatives John Magnussen and Richard Osborne and, as of the filing of this report, no representative of the company has responded.

1553791168 2019-03-28 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice # 2135536926, regarding the missing urban safety equipment (low-pressure relief valve, spark arrestor, carbon filter), the load-lines needing secured with locks and updated identification relevant to current ownership information. At the time of this inspection, the load lines still needed locks installed and none of the urban safety equipment has been installed on the production tanks has been installed. The identification has been updated to reflect current owner information. The well was not in production and the valve on the tubing was open but the 2 inch flow line valve was closed. The well, mechanical separator, and production tanks were surrounded with board on board fencing and secured with locks on the man doors. I contacted Rockefeller Oil Co. and spoke with Jenny Corrao regarding the locks for tank valves needing installed and installing the urban safety equipment on the production tanks.

1660621576 2014-07-29 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 11/22/2006. The unit is producing and production reports are current. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger-lift system and aboveground storage tank (AST) battery. The wellhead and battery are accessed via a dedicated gravel access road which travels north from Norton Parkway, to a low-lying area behind a newer residential development. The aboveground storage tank (AST) battery is located 60 feet to the south of the well in a separate board-on-board enclosure. The battery is comprised of a total of 1 steel 100-barrel production tank and 1 steel 100-barrel brine tank, and horizontal separator in a 3rd, separate, locked enclosure. Spacing from well to battery, and spacing of elements within the battery meet regulations. The load-outs are bull-plugged. The secondary containment at the battery is present but very low and not adequate to contain a release. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery which is legible and in good condition. The area surrounding the wellhead slopes to the west towards Newell Creek which is 100 feet west-southwest of the unit. I did not observe any signs of drainage or erosional deficits associated with the unit. I notified Great Plains about the containment dike deficiency.

2135536926 2016-02-18 UrbanProduction Wells Violations Noted I inspected this producing well as part of routine inspection activity. At the time of my inspection, I observed the following elements: a wellhead with mechanical plunger lift and tank battery consisting of two 100-barrel steel, low-profile storage tanks, with mechanical separator. The battery and wellhead were in in separate board-on-board enclosures. Fittings and connections are free from leakage. The production tanks did not have a spark arrestor, a low-pressure relief valve, or a carbon filter as required. The dedicated access road is in good condition. The enclosure gates were locked and secure. The load-line valves were not locked and must be secured with locks. The owner must remedy the following by 04/01/2016: secure the load-line valves and enclosure gates with locks. Supply and correctly fit the production tank with carbon filter, low-pressure relief valve, and spark arrestor.
