0018062874 2017-12-11 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: the 210-production tank and the 100-barrel brine tank were severely rusted and lost integrity, the load-line valves were not locked and secure, and the unit signage had not been updated since the change of owner that had occurred in 2016. At the time of this inspection, I observed the following: the noted deficiencies remained. In addition, there was no spark arrestor, low-pressure relief valve, or carbon filter and there was no fencing or enclosure at the wellhead or battery. The tank battery and mechanical separator had been spray-painted by vandals. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.

1457377691 2016-03-07 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. Access to the well is via a gated entry at Ravenna Road which was unmarked. At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger lift system, a tank battery comprised of one 210-barrel steel production tank, one 100-barrel steel brine tank, a mechanical separator. Fittings and connections were bull-plugged but not locked. Low-pressure relief valve, spark arrestor, and carbon filter were not present. At the time of this inspection I observed the brine storage tank to be severely corroded and failing in mechanical integrity. The owner must remedy the following by 04/08/2016: remove the failing tank from service, secure the load-line valves with locks. Place ID signage at the entry gate. Fit the new production tank with spark arrestor, low-pressure relief valve, and carbon filter as required. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.

At the time of the previous inspection, I had noted the following: the load-line valves were not secure with locks, the brine tank was perforated with numerous holes, and the tanks were missing carbon filter and spark arrestor. At the time of this inspection, the noted deficiencies remain. In addition, the signage has not been updated to reflect the change of ownership that occurred in 12/2016. I contacted John Magnussen with David Oil who indicated that the issues are being handled as they are able. I inspected the wellhead and surrounding area and found no signs of problems otherwise.
