0758195431 2017-09-01 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: the load-lines were no secured with locks, and the enclosure fencing had become broken down with missing segments and not secure. In addition, the identifying signage had remained unchanged since the previous change of ownership and had not been updated to reflect new ownership. At the time of this inspection, the aforementioned deficiencies had not been remedied. The owner has made repeated pledges to address the issues, but with no demonstrable progress.

At the time of the previous inspection, I had noted the following: the tank battery enclosure was partially broken down, with a missing section. The load-line valves remain unsecured with locks. That condition remains at the time of this inspection. Also, I noted that the ID signage reflects the prior owner and remains not-updated since the transfer of ownership in 12/2016. I contacted John Magnussen with David Oil who reported that the issues are being tended to as they are able.,
1228199983 2017-12-06 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: the load-line valves and enclosure gates were not locked, and portions of the battery enclosure were missing. At the time of this inspection, I observed the following: there were no locks at the load-line valves and battery enclosure gates. The battery enclosure was partially missing. In addition, the ID signage has not been updated to reflect the change of ownership that occurred over 12 months prior to this inspection. The tank battery gate was missing board sections and the wellhead enclosure was partially broken down. I have contacted the company on several occasions since the issuance of the compliance notice. The company has reported that problems with personnel and/or lack of funds, have prevented them from being in compliance.

The battery, separator, and wellhead are in separate board-on-board enclosures. The battery is comprised of 1 210-barrel steel production tank, one 100-barrel steel brine tank, and separator unit. There is a trailer-mount compressor unit in operation beside the separator. There is adequate secondary containment. There is a spark arrestor, low-pressure relief valve and carbon filter as required by 1501:9-9-05E. Locks and signage are present. No signs of leaks, spills or problems. Preliminary restoration passes.

The owner has not been responsive to multiple requests from the Division to remedy these issues.

At the time of this inspection, I observed the following elements: a wellhead with pump jack system in a board-on-board enclosure. A tank battery comprised of one 210-barrel steel production tank, one 100-barrel steel brine tank, a mechanical separator in separate board-on-board enclosure. The enclosure gates were unlocked and not secure. A portion of the battery enclosure was broken off. Fittings and connections were bull-plugged but not locked. Low-pressure relief valve, spark arrestor, and carbon filter was present. Fittings and c connections at the wellhead were secure and free from signs of leakage or problems. The owner must remedy the following by 04/08/2016: secure the load-line valves and enclosure gates with locks. Repair the battery enclosure fence.
