0477730684 2012-04-30 UrbanProduction Wells No Violations On 4/30/2012, I completed an urban production well inspection status check for DNR Inspector Tom Hill. My inspection found the well head was a plunger lift unit (rabbit) surrounded by a chain link fence with the proper identification posted. The lease road is in good condition and part of the land owners driveway. The tank battery contained one 100 barrel oil storage tank, one 50 barrel brine tank, and a separator surrounded by a chain link fence with the proper identification posted. I noticed no visible leaks or spills. There are no violations at this time.
0693100788 2017-11-29 Production Wells No Violations I inspected this well as part of routine inspection activity. The well and battery are located on a rural, in a wooded clearing beyond the house. At the time of this inspection, I found the following: a wellhead plunger-lift unit and associated battery in separate chain-link enclosures. The fittings and connections were secure and free from leakage or problems. Fittings and connections at the battery were secure. The load-lines were plugged and locked. ID signage was present and current. The secondary containment was intact with no signs of leakage, spills, or problems. The shared-use access road was in good condition. The well was in active production. The chart recorder was active. Production reports and insurance were current. I had noted during a previous inspection that the secondary containment drain was unsecure. At the time of this inspection, the drain was plugged and secure.
1147093632 2013-10-21 UrbanProduction Wells No Violations The producing unit consists of a mechanical plunger lift unit and above-ground storage tank (AST) battery. The battery is comprised of 1 140-barrel steel production tank, 1 50-barrel brine tank, and separator unit. Load-outs are locked. ID placard and secondary containment are good. The wellhead and battery are enclosed in separate chain-link fences. No problems noted.
1312806608 2011-08-08 UrbanPreliminary Restoration No Violations This site passes Preliminary Restoration.
1333458147 2012-03-19 UrbanProduction Wells No Violations My inspection today found the well site in good repair, but the identification sign is bleached out and is not legible. I contacted Ralph Gregory and notified him to repair/replace the identification sign.

1786718637 2017-09-13 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with plunger-lift system and associated tank battery in separate chain-link enclosures, both located on a rural residential parcel. The enclosure gates were locked and secure. The load-line valves were secure and locked. Tank lids were closed and secure. The secondary containment was intact and dry. I walked the areas surrounding the well and battery and found no signs of impacts or problems.
1845178537 2019-01-07 UrbanProduction Wells No Violations I conducted an inspection on this well. At the time of this inspection, the well was equipped with a plunger lift assembly and in production. There was a domestic line plumbed off the 4.5 inch casing with a regulator, drip bottle and meter. The valve on the domestic line was open. The well was fenced with a locked door and proper, legible identification. The tank battery consisted of one 100 BBL tank, one 50 BBL tank and a mechanical separator. Valves on the separator were open. The tank load lines were plugged and locked. The diked tank battery was fenced with a locked man door. Legible identification was present at the tank battery. The lease access was stable.
1886598791 2014-09-30 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The producing unit is currently comprised of a wellhead with mechanical plunger lift system in a locked chain-link enclosure, and tank battery is separate chain-link enclosure.
