0798753794 2017-12-13 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: The load lines were not locked and secure. At the time of this inspection, I observed the following: the load-line valves remained unlocked. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.
0819228959 2011-12-13 Preliminary Restoration No Violations This well is in Concord Township which is covered by HB-278. The well is in a secluded area on the operators property. The property/estate is gated and you have to cross Big Creek stream to access the well. The lease road is stable. This well is equipped with a pumping unit. The well head and tank battery are not fenced or landscaped. The area affected from drilling has been reclaimed and the site is stable and in good repair. The tank battery is equipped with one 210 barrel tank, one 100 barrel tank and a heat treat separator. Equipment spacing is correct. The remote location of the well and the security of the property would have allowed for the waiver of the landscaping and fencing during the permitting process. Requirements are waived at this time. If there is an ownership change HB-278 equipment will need to be installed.

1251381476 2016-03-02 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. The well and battery are accessed via a dedicated, gravel-paved lease road that enters from Williams Road. The head of the lease road is gated. At the time of this inspection, I observed the following elements: a wellhead with pump jack system and, a tank battery comprised of one 210-barrel steel tank, one 100-barrel steel tank, a mechanical separator. Fittings and connections were bull-plugged but not locked. The owner must remedy the following by 04/08/2016: secure the load-line valves with locks. Place ID signage at the entry gate.
