0098012218 2014-08-13 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 01/25/2006. The unit is producing and production reports are current. Elements of the well consist of a production wellhead with standard valve assembly and electric pump jack system and aboveground storage tank (AST) battery in separate chain-link enclosures. The wellhead and surrounding area are free of signs of leaks or other impacts. The wellhead and battery are located in a sparsely developed portion of Willoughby Twp., in a flat-lying parcel currently being used for agriculture (apple orchard). The area surround the wellhead and battery is heavily wooded. Access to the wellhead and battery are via separate gravel access roads from Dodd Rd. The battery is comprised of 1 steel 100-barrel production tank, 1 25-barrel poly brine tank, and separator unit. Spacing from well to battery, and spacing of elements within the battery meet regulations. The load-outs are bull-plugged. The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. Surface drainage from the area is towards the Chagrin River, 400 feet to the northwest. I did not observe any gully formation or other surface drainage erosional deficits. The ID placards at the wellhead and battery are in good condition.

1457382969 2016-03-07 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following elements: a wellhead with electrical pump jack system in a partial chain-link enclosure. The tank battery is comprised of one 100-barrel steel production tank, one 50-barrel plastic brine tank, and separator in a chain-link enclosure accessed from the west side of the property. Tank lids were closed and secure. Load lines valves were plugged and secure. Secondary containment was intact. Carbon filter, spark arrestor, and low-pressure relief valve were all in place. ID signage was present. I noted the following maintenance issues; some oil leakage had occurred from the stuffing box which has been partially mitigated with absorbent pads. The battery enclosure gate was closed but not secured. The existing hasp was not capable of properly securing the gate in a closed position. I contacted Alliance Supervisor Matt Postlethaite and advised about the need to complete the clean-up of the oil leak at the wellhead and repair the gate hasp.

At the time of this inspection, I observed the following: a wellhead with pump jack system and associated tank battery in separate chain-link enclosures, both located on a rural farm parcel. The enclosure gates were locked and secure. The load-line valves were secure and locked. Tank lids were closed and secure. The secondary containment was intact and dry. I did note that there was some oil leakage at the wellhead from the stuffing box-I contacted Matt Postlewaite with Alliance and advised him about the leak. He indicated that he would have the pumper respond ASAP.
