0137659812 2018-02-02 Production Wells Violations Noted I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with pump jack system and associated tank battery on a rural parcel. Production reports were current. Fittings and connections were secure. I noted the following deficiency: the 210-barrel steel production tank was severely corroded with numerous perforating holes that had rendered the tank not suitable for use. The owner must remove the tank from use immediately, and remove the tank from the battery and properly dispose the tank. In addition, the owner must place proper identifying signage at the well. The existing signage had been left from the prior owner. These activities must be completed by 4/1/2018. Contact Inspector Stephen Tompkins at 330-907-4647 with questions about this compliance notice.
0896002351 2016-01-26 Production Wells No Violations I inspected this producing well as part of routine inspection activity. The well and battery are located in a generally sparsely developed, rural area. Access to the well is afforded by a dedicated, unpaved access road. At the time of this inspection, I observed the following elements: a wellhead with pump jack system, a tank battery comprised of one 210-barrel steel production tan, one 100-barrel steel brine tank, and separator. The load-outs were bull-plugged and secure. Secondary containment is intact. I walked the areas surrounding both the wellhead and tank battery and did not observe any evidence of impact or problems associated with the production activity.
0983812478 2011-04-14 Preliminary Restoration No Violations Preliminary restoration passed.
1314798270 2011-08-31 Preliminary Restoration No Violations Lease road gated, stable. Dike, spacing, ID sign, vent hatches in compliance. Feedlines are bullplugged. Site passes Preliminary Restoration.

1961888131 2018-05-21 Production Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following deficiencies: The wellhead was missing proper identification, and the stock tank was severely corroded with numerous holes. At the time of this inspection, I observed the following: The stock tank was still in place. There was no compliant signage. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.
2084090937 2019-03-26 Production Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice # 137659812, regarding identification updated to reflect current owner information and the removal of the 210 BBL tank, due to holes that have formed throughout the tank. At the time of this inspection, the 210 BBL tank has not been removed from the tank battery location and the identification still needs updated owner information added. At the time of this inspection, the well was not in production.
