There are no enclosures. The unit ID is present and meets requirements. The loadouts are not locked. The battery secondary containment is good. I contacted Tim Reilly with Great Plains and advised about the need for locks on the loadouts.

At the time of my inspection, I observed the following elements: a wellhead with mechanical plunger lift and tank battery consisting of one 210-barrel steel storage tank, with mechanical separator. The battery and wellhead are not enclosed. Fittings and connections are free from leakage. The tank lid was closed and sealed. The dedicated access road is in good condition. The load-line valves were not locked and must be secured with locks. The owner must remedy the following by 03/25/2016: secure the load-line valves with locks. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.

At the time of this inspection, I observed the following: the noted deficiencies remained. In addition, I noted that the wellhead and battery were not in enclosures as required. I had previously contacted the company about the deficiencies and they had reported various issues regarding lack of personnel and funds preventing them from being in compliance.

The well and battery are located in a residential area along Jackson Street in Painesville. The dedicated lease road adjacent to the house at the referenced address The wellhead plunger lift system and battery are located in an open area behind houses. The wellhead and battery are not in enclosures. Connections and fittings are secure and free from signs of leakage or problems. Load-outs are plugged and secure but are not locked which was noted and reported to Great Plains at the last inspection. Areas around the wellhead and battery are snow-covered at the time of this inspection. I notified Great Plains a second time about the need for locks at the load-out. I did not observe any evidence of impact relating to on-going production activity.

1805734383 2017-04-06 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued Compliance Notice. At the time of this inspection, I found the following: Fittings and connections at the wellhead were secure and free from leakage. The tank battery was comprised of a 210-barrel steel production tank with separator unit located in the rear portion of a parcel which was largely undeveloped, but utilized for stockpiling of bricks and other debris. Load-lines remained unsecure with locks as required. The ID signage remains not updated to reflect the new owner. I contacted John Magnussen with David Oil and advised him about the continuing deficiency. He indicated that the items are being worked on as they are able. I contacted Richard Osborne Sr. with David Oil Co., and as of the date of this report, I have not received a response.
