0469665664 2012-05-04 Production Wells No Violations On 5/4/2012, I completed a follow-up production well inspection for DNR Inspector Tom Hill. My inspection found all the oil contaminated soil/gravel had been removed, new gravel had been spread within the diked tank battery and lease road, the dike wall was raised on the south and east sides, and the 4 concoil was cleaned of all salt and oil residue. There was no identification sign posted, I informed DNR Inspector Tom Hill of all my findings.

1032022588 2018-07-24 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. At the time of this inspection, I noted the following: a wellhead with plunger-lift system and associated tank battery is a clearing adjacent to the road. The access road was gravel-paved and well drained. Fittings and connections at the wellhead and battery were secure with the following exception: there was a small leak of gas from the flow-controller, which I notified the owner about. They indicated they would have the tender evaluate as soon as possible.

1335553935 2012-04-25 UrbanProduction Wells No Violations My findings during this inspection are as follows: Well site needs an identification sign posted. Well spacing is not correct. ORC 1501 requires 50 feet between the production tank and the wellhead and 50 feet between the wellhead and the separator. There was 399 between the production tank and the wellhead. There was 222 between the wellhead and the separator. There is a one inch PVC drain pipe installed in the north side of the tank battery dike. The pipe has a valve on it, the valve was open. Valve needs to be closed at all times when not attended. There were empty motor oil containers in side the dike. Containers need to be removed. There is a compressor inside the tank battery. The oil has been changed recently on the compressor. The operator allowed used oil from the compressor to drain onto the tank battery floor. The used oil filter from the compressor motor was also laying on the tank battery floor. Compressor needs temporarily moved and all oil contaminated soil removed from location. The 100 barrel production tank had a 6 X 3 oil stain under the load valve. All oil stained soil from around and under the production tank needs removed from location and replaced with clean clay. There is oil and brine staining and scalding around the wellhead. Contaminated gravel needs removed from around the wellhead and replaced. There is a barren area approximately 20 X 15 east of the wellhead. There is salt staining on the ground in that area. Wildlife has been disturbing the soil for salt. Vegetation is stressed around the barren area. The area appears to be the remains of a work pit. Contaminated soil and pit material needs removed from location and replaced with clean soil. Soil needs to be seeded. There has not been any HB-278 safety equipment installed on the tank battery equipment. The 100 barrel production tank is not the correct tank for HB-278. The production tank does not have hatches that seal. Operator has been notified by phone
1362593835 2013-03-01 Preliminary Restoration No Violations The producing unit consists of the following elements: A production wellhead and tank battery. The production wellheadis fitted with a concoil unit, and is not enclosed. The tank battery is comprised of 1 100-barrel (BBL) tank, 1 50 BBL tank, mechanical separator, skid-mounted compressor unit, and associated piping and appurtenances, not enclosed. I did not observe any leaks, releases, or problems. The site and lease road are in good condition.
