0201118603 2016-02-16 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection I observed the following elements: A wellhead with gas-fired pump jack system, a mechanical separator, and a tank battery system located adjacent to the existing parking and drive areas. Access to the wellhead is via the shared, paved truck delivery driveway. Fittings and connections at the well and battery were secure and free from evidence of problems or leakage. Load line valves were locked and secure. I walked the areas surrounding the well and battery and did not observe any evidence of impact relating to the production activity.
0239197259 2012-10-26 UrbanProduction Wells No Violations The unit consists of a 2-tank battery with mechanical separator. The spacing is in accordance with the rule, and the valves and mechanicals are in good repair. The loadout valves need locked. I did not observe any signs of leaking product or gas vapors. The wellhead is fitted with a pumpjack. The tank battery dike is missing in parts, as is the dike around the mechanical separator. Wellhead is located adjacent to storm drain and presents a risk of release to surface water. Im recommending a containment dike for the wellhead . I called Tim Reilly of Great Plains and advised him about the need for locks, dike repair and construction. I indicated that the requested date for completion is 12/31/2012.

1533680404 2017-09-06 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. The wellhead pump jack system and associated tank battery are located on a parcel occupied by a light industrial facility. At the time of this inspection, the load lines and enclosure gates were locked and secure. Fittings and connections at the wellhead and battery were secure and free from signs of leaks, spills, or other problems with the following exception: I noted some oil leakage at the flow-line near the separator unit. I notified John Magnussen with Rockefeller Oil about the leakage, and he indicated that he would respond and evaluate ASAP. I noted that the ID signage had not been updated to reflect the change of ownership that occurred in 2016. I have previously spoken with John Magnussen about the signage change over, to which the response has been that issues such as this will be handled as the owner is able.
1711311566 2014-12-04 UrbanProduction Wells No Violations I inspected the producing unit as part of routine inspection activity. The wellhead pump jack and tank battery are located at the end of the warehouse access road for the MCSign Co. The surrounding areas are comprised primarily of asphalt paving and gravel. Fittings and connections at the wellhead and battery are secure with no signs of problems or leakage. Secondary containment and signage are present.
1842562302 2017-12-08 UrbanProduction Wells No Violations I insepcted this producing well location as part of routine inspection activity. The well and battery are located in a open area, in an otherwise commerical and light industrial area. Fittings and connections at the wellhead pumpjack and battery were secure with no signs of leakage or problems. The well was in active production with active chart record. The load-line valves were locked and secure. The ID signage was current and compliant.
