0359576855 2017-06-29 Production Wells No Violations I inspected this producing well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with pump jack system and associated tank battery located on a rural farmstead property. The dedicated lease road was dry and in good condition. Fittings and connections were secure with the following exception: I noted a 2-inch cross-over line in the tank battery was corroded and may be leaking. The there was a small area of oil impact within the secondary containment berm. I notified both the contract pumper (Matt Sisson) and the well owner about the maintenance items. He indicated that repairs were in progress.

0849031017 2015-09-15 Production Wells Violations Noted I inspected this producing well as part of routine inspection activity. The wellhead and battery are located on the periphery of a farm field. The area around the well and battery is comprised of meadow grass and some shrubbery. The unit is accessed via a short, unpaved road which enters from Girdled Road. Fittings and connections at the well and battery are secure and free from signs of leakage or problems. At the time of this inspection there is no current, accurate identification. In addition, the wellhead pump jack has become overgrown with vegetation and requires proper maintenance. The well underwent a change of ownership in July and the new owners have failed to make the appropriate updates to the signage. There is no working emergency contact number. I have contacted the well owner and advised that the signage deficiency must be remedied in 30 days.

The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. I did not observe any leaks, spills, or other signs of contamination. There is no fence enclosure so the loadouts need locked. I called Steve Chomos at Broadstreet . He indicated that the locks would be taken care of as the pumper makes his rounds.

At the time of this inspection, I observed the following: one wellhead with pump jack system. Fittings and connections at the wellhead are secure. The associated battery is comprised of two 210-barrel steel production tanks, one 100-barrel brine tank, and associated separator. The load-lines were plugged and secure. The secondary containment was intact. The tank lids were closed and secure. The dike drain was secure. I observed some corrosion at the base of one of the 210-barrel tanks, at the crossover drain connection. I advised Matt Sisson, well tender for M & R, that repair or replacement of this tank is warranted. The signage which I had observed to be missing emergency contact information has been updated.

At the time of this inspection, I noted the following: a wellhead with pump jack system and associated tank battery on a larger, rural, farmstead. Fittings and connections at the well and battery were secure and free from leakage or problems, with the following exception. I had noted during a prior inspection that there might be on-going leakage occurring from the cross-over line. There was oil-impacted ballast material along the run of the cross-over indicating a prior spill or an on-going leak. I notified Bill Fitzmartin that the owner should evaluate the cross-over, and the oil must be remediated.
