0155802052 2017-10-05 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: The wellhead pump jack and associated tank battery located in on an otherwise agricultural parcel. The load lines were plugged and secure. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery which are comprised of dense, tall grasses and wooded areas and found no evidence of problems or impact to the surrounding greenscape attributable to the on-going production activity. I noted that the former 210-barrelproduction tank had been disconnected but remained in the battery. I notified the owner that the discarded tank must be removed from the battery within 30 days per rule. The battery is shared with the Fidel #3 and #4 wells.

The battery is comprised of 1 210-barrel steel production tank, 1 100-barrel brine tank, and 2 separator units. The battery is apparently shared with the #862 and the #939 units, but the ID placard is not readable. The wellhead is not enclosed. The battery is in a chain-link enclosure. The load-outs are bull-plugged. The tank lids have been left open. Secondary containment is good. No other problems noted. I have contacted Broad Street Energy about the signage requirements and requirement to close and seal lids when unattended.

1427898717 2015-04-01 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. The wellhead and battery are located in a farm field on the west side of County Line Road.. Access is from a dedicated access road from County Line Rd. behind the adjacent ODOT garage. The wellhead is comprised of a pump jack system. Fittings and connections are secure. The tank battery is comprised of 1 210-barrel steel production tank, 1 100-barrel brine tank, and separator units in a separate chain-link enclosure. Elements of the system are in older and worn but fittings and connections are secure and free from signs of leakage at the time of this inspection. The load-outs are plugged and secure. ID signage is missing. Areas surrounding the wellhead and battery are reasonably well drained and free from signs of adverse impact from the on-going production activity associated with this well. The battery is shared with the #2-0862 (Fidel #3) well which is located near County Line Road. I notified Broad Street about the missing signage.

At the time of this inspection I noted the following; The well was in active production with active chart record. I noted that the well ID signage had become faded and unreadable. I advised Bill Fitzmartin with M&R Investments about the need for maintenance. He indicated they would remedy the signage as soon as possible.
