0316207553 2017-09-06 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issues: there had been leakage from a hole in the production tank. At the time of this inspection, the leakage had been repaired. I noted, however, that the load-line valves and enclosure gates were not locked and secure, the tank spacing does not meet requirements, and the ID signage has not been updated to reflect the transfer of ownership that had occurred over six months prior. I have advised the owner that I will issue a separate compliance notice regarding these deficiencies.

1388310162 2016-02-16 UrbanProduction Wells Violations Noted I inspected this well location as part of routine inspection activity. At the time of this inspection I observed the following elements: A wellhead with gasoline-fired pump jack system, a mechanical separator, and a tank battery system located adjacent to the existing parking and drive areas in separate enclosures that were locked. The battery is comprised of two 100-barrel fiber-reinforced plastic (FRP) tanks. Access to the wellhead is via the shared, paved truck delivery driveway. Fittings and connections at the well and battery were secure and free from evidence of problems or leakage. Load lines were plugged and secure. I noted a small pinhole in one of the tanks which was not actively leaking but showed signs of past leakage. I walked the areas surrounding the well and battery and did not observe any evidence of impact relating to the production activity. The owner must remedy the following by 04 /04/2016: repair/replace the leaking tank. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.

1899185331 2017-04-19 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for the following: leaking from the production tank. At the time of this inspection, I observed the following: There had been no further leaking from the storage tank. I did note that the load-line valves were not secured with locks as required. In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.
1992352978 2012-10-26 UrbanProduction Wells No Violations The unit consists of a 2-tank battery with mechanical separator in a 6-foot chain link fence. The spacing is in accordance with the rule, and the valves and mechanicals are in good repair. The dike is adequate. The loadout valves need locked or the gate needs a lock. I did not observe any signs of leaking product or gas vapors. The wellhead is fitted with a pumpjack and is enclosed in a 6-foot wooden fence. I called Tim Reilly of Great Plains and advised him about the need for locks. I indicated that the requested date for completion is 12/31/2012
