At the time of this inspection, crews were blowing down the well and loading the well borehole. A circulation pit was established with a open-top steel tank located adjacent to the wellhead with 2-inch discharge line secured with tie-down chains. Prior to placement of plugs, the well was quiet with stable conditions. Plugging activities conducted included placement of the bottom formation (Clinton) plug.

0618620365 2014-07-29 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 10/19/2003. The unit is producing and production reports are current. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger-lift system. The wellhead and battery are located at the end of Tin Man Parkway, in an open area within a primarily light industrial and corporate park. There are no signs of leakage or problems at the wellhead. The battery is comprised of a total of 1 100-barrel fiber-reinforced plastic (FRP) production tank, 1 50-barrel steel brine tank, and horizontal separator. The battery is located in a chain-link enclosure which is gated and locked. The load-outs are bull-plugged. The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery which is legible and in good condition. The wellhead is not enclosed, but is isolated. Areas around the wellhead are utilized for stockpiles of gravel and other agregate material. I did not observe any signs of impact associated with the well in the surrounding areas.
0711460156 2016-02-16 UrbanProduction Wells Violations Noted I inspected this well location as part of routine inspection activity. At the time of this inspection I observed the following elements: A wellhead with mechanical plunger lift system, a mechanical separator, and a tank battery system. The battery was comprised of one 100-barrel fiber-reinforced plastic (FRP) production tank and one 50-barrel steel brine tank in a chain-link enclosure. Access to the wellhead is the unpaved drive and access road for a materials storage yard. Fittings and connections at the well and battery were secure and free from evidence of problems or leakage. The wellhead was not enclosed. The enclosure gate at the battery has been partially broken off the hinges and needs repair of replacement. I walked the area surrounding the wellhead and battery and did not observe any evidence of impact or problems associated with the production activity. The owner must remedy the following by 04/04/2016: Repair the broken fence that is a safety hazard. Contact Inspector Stephen Tompkins at 330-907-4647 with questions regarding this inspection.
0939727637 2012-10-26 UrbanProduction Wells No Violations The unit consists of a 2-tank battery with mechanical separator in a 8-foot wooden fence. The spacing is in accordance with the rule, and the valves and mechanicals are in good repair. The dike is adequate. The loadout valves need locked or the gate needs a lock. I did not observe any signs of leaking product or gas vapors. The wellhead is fitted with a pumpjack and is enclosed in a 6-foot wooden fence. I called Tim Reilly of Great Plains and advised him about the need for locks. I indicated that the requested date for completion is 12/31/2012.
1026603714 2016-08-29 Plug / Plug Back No Violations I inspected this well location pursuant to a call from Gibson Energy that his crews were at this location to begin dismantling the production equipment in anticipation of plugging and abandonment. Reportedly, ownership of the well is transferring from Great Plains Exploration to the property property owner. At the time of this inspection, transfer had not occurred and no permit to plug had been issued. I met with Ron Gibson and he indicated that work was not scheduled to occur until next week, so issues regarding well ownership and plugging permit could be resolved.

1477506491 2016-10-25 Plug / Plug Back No Violations I was on location to witness on-going plugging and abandonment activity being performed by RA Gibson under contract to Capannoni USA LLC. At the time of this inspection, crews completed placement of a formation plug at the Big Lime and a combined bottom surface and top surface plug without incident or problem. At the completion of this inspection, the top surface plug was stable with no signs of gas production or fluid losses.
1479486134 2016-11-18 Final Restoration No Violations I inspected this well location for evaluation for Final Restoration. Plugging and abandonment activities were completed on 10/25/2016 At the time of this inspection, the former location of the well and battery had been graded to match the surrounding grade and the area is level and stable with no signs of erosional deficits. A waiver has been submitted to allow for the 2-inch flow-line to remain in place. All other produciton equipment has been removed. No further activity is needed. Final Restoration is passed.
