0652162048 2016-07-21 Final Restoration No Violations I inspected this well location as a follow-up to plugging and abandonment activities which were executed on 03/04/2016. At the time of this inspection, the former well was located within the subsurface vault structure and sealed with ID as prescribed. The was no indication of leakage or problems. The former production appurtenances located on the building exterior have been removed and the area restored. The former supply line was cut and sealed and abandoned in place per the request of the landowner. The well vault structure and lid will remain in place. No further activities needed. Final Restoration passes.
1021299658 2016-03-03 Plug / Plug Back No Violations I inspected this well location as part of planned plugging and abandonment activity. At the time of this inspection, I met with Paul Mihalik who hass been contracted by the previous proper owner to plug the well. The well is located near the rear portion of the existing car dealership in a subsurface vault with a round, bolt-down style man lid. The well is comprised of a 5.625-inch casing riser with a c-clamp style lid. There is a 4-inch diameter sleeve-style liner casing within the annulus. The depth to bottom has been measured at 850 feet. There is a former supply-line connection from the well to the car facility. At the rear exterior wall, there is a drip tank and valve connection and supply line into the building. Gas pressure at the well was 25 PSI. Plugging will commence within the next several days anf an evaluation of the liner and attmept to remove. A pluggging strategy will then be developed.
1431955055 2015-05-15 Administrative Inspection No Violations I inspected this well location relative to a pending Chiefs Order. The well is located in a primarily commercial district on Mentor Ave. The well is located adjacent to the existing car dealership building. Visible elements consist of a 1-inch supply line connected to the back wall of the building , with a drip tank (10-gallon) and motor valve. The well casing is buried and not visible. The elements are secured to the back exterior wall of the building and painted white. There is a gate valve on the supply line which is closed at the time of this inspection. I did not observe any signs of leakage or problems associated with the visible elements of the well.
1439303826 2015-08-10 Administrative Inspection No Violations I inspected the reported location of this well in an attempt to confirm whether the well has been plugged and abandoned. At the time of this inspection, I observed a man-lid in the parking area located in the rear portion of the auto dealership. Upon removal of the lid, I observed a wellhead comprised of a surface casing riser with a 2-bolt lid, and with two visible supply-line connections. The vault is partially filled with water obscuring any further inspection. There is a drip tank and other supply appurtenances attached to the back exterior wall of the existing building. The supply line enters the building in this area. It is not clear on inspection, to what areas gas is, or was historically, supplied by the well. The supply valve from the drip tank is closed at the time of this inspection. At the conclusion of this inspection, I replaced the man-lid cover to the well vault.
