0608202361 2018-05-04 Production Wells No Violations I inspected this well location as an evaluation of current condition and status pursuant to a recent change in ownership. At the time of this inspection, I found the following: a wellhead with plunger lift in a small chain-link enclosure. There was a newer domestic tie-in system with positive-shut off (PSO) drip and regulator. The domestic system was in good repair. I noted that no annual production report had been filed for 2017 and contacted the new owner on two separate occasions, and had not received a response at the time of this report filing.
1353952405 2012-11-05 Production Wells No Violations The wellhead is fitted with a mechanical plunger lift and is located 300 feet north of the tank battery. The wellhead is enclosed in a 6-foot, chain link fence and is in acceptable condition. T he tank battery consists of a 2-tank battery (TB) with mechanical separator, enclosed in a 6-foot fence. The spacing is in accordance with the rule, and the valves and mechanicals are in good repair. The containment dike is good. I did not observe any signs of leaking product or gas vapors. No violations noted.
1412606823 2014-10-03 Production Wells No Violations I inspected this producing unit pursuant to a complaint regarding a possible leak. The wellhead is located near a new home which is under construction at the time of this inspection. The landowner had reported a suspected gas leak from a fitting at the wellhead and pursuant to this complaint. Autumn Ridge has shut in the well pending evaluation. I met Ron Slother with Autumn Ridge, at the well location. We discovered gas leaking from a 1/4-inch copper fitting when pressured up. Ron made repairs to the fitting. I also observed the mechanical plunger lift system to be significantly corroded. Im advising Autumn Ridge to repair or replace the plunger lift unit.
1446127753 2015-10-29 Production Wells No Violations I inspected this well as part of routine inspection activity. The well is comprised of a mechanical plunger-lift system in a small chain-link enclosure. There is a domestic metering tie-in which has been added since the previous inspection. Fittings and connections at the wellhead are secure and free from leakage. The tank battery is comprised of 1 210-barrel production tank and 1 140-barrel brine tank, with a mechanical separator in a chain-link enclosure. Fittings and connections at the battery are secure. Access to the battery is afforded via a short, unpaved road which enters from Balch Road. Secondary containment is intact. Signage is present. I walked the area surrounding both the well and the battery and did not observe at evidence of impact relating to the on-going production operation at the time of this inspection.
1514914807 2018-01-02 Production Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, the enclosures at the wellhead and battery were locked and secure. The fittings and connections at the wellhead were secure with the following exception: there was gas leaking from a copper fitting at the wellhead and I notified Autumn Ridge (Jesse Malner) about the leak. He indicated that he would tend to it immediately.
1516208648 2018-01-12 Production Wells No Violations I inspected this well location as a follow-up to a previous inspection in which I had noted that gas was leaking from a copper compression fitting at the wellhead and had subsequently notified the owner. At the time of this inspection, I observed that the leak had been fixed. I found no other issues or problems.
1554472464 2019-04-05 Production Wells Violations Noted I conducted an inspection on this well as a request from landowner Dan Grein. Mr. Grein is considering purchasing the well for domestic purposes and wanted an inspection conducted on the well location. At the time of this inspection, the well was equipped with a plunger lift assembly and in production. The gate valve on the tubing and the valve on the 2 inch flow line were open. There was a domestic line plumbed off the 4.5 inch casing that has been disconnected. The well was fenced. The tank battery was comprised of one 210 BBL tank, one 100 BBL tank and a mechanical separator. Valves on the separator were open and the chart on thes sales line showed current production. The 210 BBL tank had some staining at the base, near the heater tube from a previous release. The tank has been disconnected from the other production equipment and has been emptied. No current leaks were observed at the tank battery or well, at the time of this inspection. The diked tank battery had identification, but still contained the previous owner information and needs to be updated to include current owner information. The lease access was stable. I contacted STMX Gas Inc. and left a message regarding the identification needing updated.
1688787302 2009-10-16 Production Wells No Violations An inspection was conducted to obtain a fix on the tank battery location. There were no violations at the time of inspection.
1821562536 2014-10-29 Production Wells No Violations Inspected this producing unit as a follow-up to any earlier complaint regarding a possible leak of gas at the wellhead and over-all condition of the unit. Gas had previously been leaking from a 0.25-inch pressure fitting at the wellhead. That has been repaired. The mechanical plunger lift system is aged and has visible corrosion but is functioning. Ive recommended to Autumn Ridge that they repair or replace the rusting elements. Ive also requested that an ID sign be placed at the wellhead. The gate to the enclosure is now locked. I did not observe any signs of leakage or problems. Similarly, I inspected the tank battery for signs of leakage or problems and found none. The chain-link enclosure is intact and locked. ID signage is present. The secondary containment dike is intact. Valves and connections are intact and free from signs of leakage. The tanks are rusty but the overall integrity appears intact.
1927552380 2012-11-05 Production Wells No Violations The unit consists of a 2-tank battery (TB) with mechanical seperator enclosed in a 6-foot fence. The spacing is in accordance with the rule, and the valving and mechanicals are in good repair. I did not observe any signs of leaking product or gas vapors. The wellhead is fitted with a mechanical plunger lift and is located 800 feet to the north. The tank battery dike is adequate. There is no ID at the wellhead. The gate is unlocked. I spoke with Jesse Mulnar at Autumn Ridge regarding the ID and lock. He indicated that the well the battery was about to be upgraded.
1989528275 2014-08-14 Production Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 08/17/2007. The unit is producing and production reports are current. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger-lift and aboveground storage tank (AST) battery in separate chain-link enclosures. The wellhead and surrounding area are free of signs of leaks or other impacts. The wellhead and battery are located in a wooded and agricultural area. The area surround the wellhead and battery is heavily wooded. Access to the wellhead and battery are via The battery is comprised of 1 steel 210-barrel production tank, 1 100-barrel steel brine tank, and separator unit. Spacing from well to battery, and spacing of elements within the battery meet regulations. The load-outs are bull-plugged. The secondary containment is present at the battery. I did not observe any signs or evidence of leakage, spills, or problems. I did not observe any gully formation or other surface drainage erosional deficits.
