0238132480 2012-11-02 Production Wells No Violations The unit is comprised of a 2-tank battery (2 210-barrel production tanks). There are four in-ground concrete, brine tanks located adjacent to the production tanks. The wellhead includes a mechanical plunger lift located remotely, across the road. The tank battery is shared with the Lassnick # 4 Unit. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery but there is no ID at the wellhead. I did not observe any leaks, spills, or other signs of contamination. No violations noted.
1047035666 2016-01-28 Production Wells No Violations I inspected this producing well as part of routine inspection activity. The well and battery are located on the grounds of a former commercial campground facility which is no longer functional. Access to the well is afforded by the former unpaved campground access road which enters from Seeley Rd. At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger lift system, a tank battery comprised of two 210-barrel poly production tanks, a series of four in-ground concrete brine tanks, and separators. The battery and well are in separate chain-link enclosures. The battery is shared with the Lassnick #4 (2-0838) which is located across Seeley Road. The load-outs were bull-plugged and secure. Secondary containment is intact. I walked the areas surrounding both the wellhead and tank battery and did not observe any evidence of impact or problems associated with the production activity.
1241879418 2012-11-01 Production Wells No Violations The wellhead is fitted with a mechanical plunger lift and is located in a wooded portion residence. The well is located remote from the tank battery, which is shared with the Lassnick # 4 unit. The well is in good condition and is enclosed within a 6-foot chainlink style enclosure. I did not observe any signs of fluid or gas leaks. The lease road is in good condition. There is a locked gate at the head of the lease road. The tank battery (TB) consists of two 100-barrel production tanks, and a series of 4 concrete, in-ground brine tanks. Both units are producing.
1513783073 2014-10-29 Production Wells No Violations I inspected this location has part of routine inspection activity. The well is producing and production records are current. The fittings and connections at the wellhead are free from signs of leakage or other problems. Fittings at the tank battery are free from signs of leakage or problems. The load-outs are bull-plugged. Secondary containment is intact. ID Signage is present.

1976153689 2017-09-25 Production Wells No Violations I inspected this well location as part of routine inspection activity. The well and battery are locate on a larger, rural parcel, occupied by a campground operation, which was no longer in operation. At the time of this inspection, I observed the following: the fittings and connections at the well and battery were secure. Load-line valves were plugged and secure. I noted that the ID signage remains unchanged since the change in ownership that occurred in 2016. Numerous calls to the owner about the issue with signage at this well as well as many other wells have gone unheeded.
