0083305132 2014-05-06 UrbanProduction Wells No Violations The producing unit is comprised of a wellhead with mechanical plunger-lift system and an above-ground storage (AST) battery in a chain-link enclosure. The battery enclosure is partially torn town. The battery consists of 1 210-barrel steel production tank and 1 100-barrel steel brine tank, and 2 horizontal separators. The battery is shared with the #1111 unit. There is no ID at the wellhead. I am notifying Broad Street regarding the need for correct ID at both wellheads.
0129055332 2017-02-13 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with plunger lift system and associated tank battery located on a larger, farm parcel on River Road. Fittings at the wellhead including flow-line connections and stuffing box fittings were secure and free from leakage. The tank battery was comprised of one 210-barrel steel production tanks and one 100-barrel steel brine tank. The battery is shared with the Rogers #2 well (#2-1111). The secondary containment was present and intact. The visible system connections were secure and free from leakage. Adjacent to the battery was a tin-sided shed with a compressor system for the wells. I walked the areas surrounding both the wellhead and battery which were comprised of a gently sloping (to the south) pasture and open farm field. I did not observe any signs of impact. The access road was mostly well drained with some pot-holing typical of a shared-use farm road.
1290806225 2017-08-07 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. The wellhead plunger lift and associated tank battery (shared with the #2-1111) are on a larger farmstead property. Fittings and connections at the wellhead and battery were secure. There was no independent ID signage at the wellhead, which is needed owing to the two wells shared by the battery. I contacted and notified the well tender about the need for signage.
1737597733 2019-02-01 UrbanProduction Wells No Violations I conducted an inspection on this well. The well was equipped with a plunger lift assembly and in production, at the time of this inspection. The tank battery was comprised of one 210 BBL tank, one 100 BBL tank and two mechanical separators. Valves on the separators were open. The diked tank battery was fenced and shared with well 2-1111. Legible identification was present at the well and tank battery. There was a compressor station next to the tank battery location. The lease access was stable and a shared residential driveway.

At the time of this inspection, the fittings and connections were secure with no signs of leakage or problems. The well was not in active production. The battery is shared with the Rogers #2 well located on the same parcel. There was no active chart record. The load-lines were secure and plugged. I walked the areas surrounding the well and battery and found no signs of problems. The gravel-paved access road was in good condition with access to both the battery and the wellhead beyond.
