0584695234 2017-12-06 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued compliance notice for the following: An uncontrolled release of oil had occurred and resulted in oil impact to the areas immediately surrounding the well and traveled a short distance along a farm drainage culvert. At the time of this inspection, the drainage culvert was free of oil impact. An approximate 1,500 sq. ft. area surrounding the wellhead remained oil-impacted and denuded of vegetation. I have previously advised the company that they must remove the remaining oil-impacted material and restore the area. I have contacted the company on several occasions since the issuance of the compliance notice. The company has reported that problems with personnel and/or lack of funds, have prevented them from being in compliance.
0645945401 2017-11-17 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a release of oil that had occurred on 1/5/17. The release had impacted. At the time of this inspection, there was an area approximately 1,500 square feet, around the wellhead, comprised of oil, oil-impacted soils, and dead vegetation. There has been no apparent efforts by Rockefeller Oil to clean-up the area, remove oil-impacted, soils, or restore the land surface. I contacted John Magnussen and explained the deficiencies. I called Richard Osborne and was told he was out of town. I left a message asking that he return my call.

The tank battery is equipped with one 210 barrel production tank and a mechincal separator. The well is equipped with a plunger lift system. The lease road has been rebuilt. I sent an e-mail to Great Plains requesting explanation on how they have ownership of the well and how they can be producing it. I also have a call into our permitting office in Columbus.
1417294002 2018-04-12 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued Compliance Notice for a leaking fitting at the separator, along with severely corroded fittings associated with the flow-line system downstream of the separator. At the time of this inspection, I noted that the leak at the hammer union fitting had been repaired, but the corroded flow-line fittings and piping remained in place. The owner has made pledges to fix the items but no action had been taken at the time of this inspection.
1456866383 2016-03-01 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, I observed a wellhead with mechanical plunger lift system and tank battery comprised of one 210-barrel steel tank. The tank exhibits signs of damage relating to a previous lightening strike. The top of the tank is distorted outward. The overall integrity of the tank appears intact but I will recommned to the owner/operator that the tank be replaced. At this inspection, the well was shut in. During an earlier inspection, I had observed gas leaking from a 2-inch hammer union fitting at the mechanical separator. I had also observed several sections of piping run and fittings at the wellhead and at the battery to be severely corroded. I made notification at that time to the owner about the need to replace the deteriorated piping and fittings. Since that time, the well has been shut-in. The repairs must be made before the well can be returned to service, and by no later than 04/01/2016. Contact Inspector Stephen Tompkins with any questions about this inspection, at 330-907-4647.
1483994271 2017-01-05 UrbanProduction Wells No Violations I arrived at this well location to perform a routine inspection . Upon arrival for this inspection, I noted the following: a hammer-union fitting at the 2-inch flowline adjacent to the wellhead was actively leaking oil on to the surrounding land surface, impacting an area of approximately 15 feet by 15 feet. . The leaking oil was draining into a shallow, water-filled surface water drainage channel that traversed the site from west to east. At the approximate location of the tank battery, the drainage channel turns to the north and continues northward for several hundred feet where is drains to low-lying areas to the north. The ground surface was covered with several inches of snow and ice obscuring the limit and extent of the oil. I walked the drainage in the down-gradient direction for several hundred feet, and dug post-holes in the snow at random intervals. Based on this assessment, the extent of oil appeared to be limited to 100 feet down gradient of the wellhead, along the drainage. The drianageway was partially filled with water, with oil floating on the surface. Upon discovery of the leak, I contacted John Magnussen with Rockefeller Oil, who responded to the location within 30 minutes. I also notified the ODNR Division of Oil and Gas, Emergency Operations and Response Duty Officer. Upon arrival, John and other crew closed valves and successfully stopped the leak. I estimated that between one and two barrels of oil were released from the leaking fitting and the spill appeared to be restricted to land-based receptors, with no signs that oil had impacted navigable waters or streams. As part of the initial response, Rockefeller Crews vacuumed up available free oil from the impacted areas and created a weir dam structure down gradient of the release to capture any potential oil-impacted drainage. Sorbent booms were placed at the cut-off dam, and oil sorbent peat material was applied to the remaining areas after free oil removal. The crews planned to return in sever
1491495403 2017-04-06 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued Compliance Notice issued pursuant to an uncontrolled release of oil from the wellhead. The release occurred on 01/05/17. Initial remedial efforts were conducted to mitigate the release and recover free oil from the drainage way. However, large areas of oil-impacted soils remain. I have attempted to contact Rick Osborne, with no response to repeated calls. I contacted representative John Magnussen who indicated efforts were on-going but delayed due to lack of personnel and equipment.

At the time of this inspection, the well owner has commenced clean-up activates but several areas of oil-impacted surface soils and vegetation remain covered in crude oil and are awaiting mitigation. I contacted John Magnussen with Rockefeller Oil and advised him that additional mitigation would be needed. He advised me that the activity would be commenced as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.
1505234539 2017-09-12 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previous inspection and associated compliance notice issued for the following issue: a malfunction at a wellhead fitting resulted in an uncontrolled release of oil into low-lying areas and a seasonal drainage culvert surround the well. Following the release, the owner implemented limited remedial measures in which some portion of the oil was recovered. However, at the time of this inspection, two areas of approximately 500 square feet each with residual oil impact remain. Repeated calls to the owner have not produced any demonstrable action or results.
1658593823 2017-01-20 UrbanProduction Wells Violations Noted I inspected this well location as a follow-up to a leak of oil that had been discovered on 01/05/2017. At that time, it was estimated that one to two barrels of oil had leaked from a hammer-union fitting at the wellhead and impacted the nearby surface soils and entered a shallow drainage ditch. Initial response action conducted by Rockefeller Oil Co. consisted of collection of free-standing oil on the ground and in the drainage ditch and construction of a cut-off trench with weir dam. At the time of this inspection, the weir dam is functioning to stop any off-site migration of free-phase oil in the drainage ditch but the remaining oil-impacted areas have not been addressed. The well owner must complete site remediation which must include removal and proper disposal of all oil-impacted soils and vegetative matter from the location. At the time of this inspection, the well had been shut-in and the leaking fitting isolated. Free oil from well had been vacuumed from the land surface and a cut-off trench with weir dam constructed along the drainage pathway to prevent any off-site migration. At the time of this inspection, the area which had been impacted by the leak was stable with no observed migration of oil away from the original limits of the release.

The well and associated battery are located at the end of a shared-use, unpaved farm road. The well is comprised of a plunger lift system. The battery consists of one 210-barrel production tank and vertical separator located in close proximity to the wellhead. At the time of this inspection, there was a small, audible leakage of gas from a hammer union fitting. The production tubing at the hammer union is corroded and needs replaced. I also noted that the 2-inch line at the vertical separator is corroded and needed replacement. I notified Great Plains about the maintenance issues. Otherwise, fittings and connections at this unit are secure. Secondary containment is intact. ID signage is present.
