At the time of this inspection, the load lines and enclosure gates were locked and secure. Fittings and connections at the wellhead and battery were secure and free from signs of leaks, spills, or other problems. I noted that the ID signage had not been updated to reflect the change of ownership that occurred in 2016. I have previously spoken with John Magnussen about the signage change over, to which the response has been that issues such as this will be handled as the owner is able. |

I indicated that the requested date for completion is 12/31/2012. |

During my inspection, the following activities were performed: On 4/25/2018 , The 4.5-inch casing was mechanically ripped at 1,844 feet. Seven joints of nominal 43.3 casing were removed and circulation established. A Marcellus/Big Lime/Oriskany formation plug was set through 4.5-inch casing from 1540 to 1240 feet. Twenty-six joints of 4.5-inch casing were then removed, circulation established, and a combined bottom surface/top surface plug was set from 414 to surface. During placement of the surface plug, it was noted that there was some loss of fluid, and additional Class A cement was added, with loss-circulation material. At the completion, the top surface plug was allowed to stabilize for 30 minutes. The plug was noted to drop approximately 65 feet. The plug was allowed to stabilize overnight, until 04/26/18. During site activities, I monitored for the presence of combustible gas, including methane and hydrogen sulfide, and observed no detected readings. |

The wellhead and battery are located at the end of Red Oak Drive, which forms a cul-de-sac. There are no signs of leakage or problems at the wellhead. The battery is comprised of a total of 2 steel 100-barrel steel low-boy ASTs (1 production and 1 brine tank), and horizontal separator. Spacing from well to battery, and spacing of elements within the battery meet regulations. The battery is located in a separate board-on-board enclosure which is gated and locked. The load-outs are bull-plugged. The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition. |
