The wellhead is located in a common area within a cluster-home style community. The well was comprised of a plunger-lift system in a small chain-link enclosure. The associated battery is located east and adjacent to the development, in a separate chain-link enclosure. The enclosure gates were locked and secure. The load-line valves were plugged and secure. Fittings and connections at the well and battery were secure and free from signs of leakage or problems. There was an active chart record in the recorder box. Production records were current. The ID signage was present and updated. I walked and inspected the areas surrounding the well and battery and found no signs of impact or degradation.

The battery, separator, and wellhead are in separate board-on-board enclosures. The battery is comprised of 1 210-barrel steel production tank, and separator unit. There is adequate secondary containment. There is no safety equipment as required by 1501:9-9-05E. Locks and signage are present. No signs of leaks, spills or problems.

During this inspection, the well was observed to be equipped with a plunger lift assembly and not in production. The well was fenced with a locked man door. The tank battery was comprised of one 210 BBL tank and a mechanical separator. The diked tank battery was fenced with legible identification. The fence was locked and the lease access was stable.

At the time of my inspection, I observed the following elements: a wellhead with mechanical plunger lift system in a chain-link enclosure which is locked. The enclosure is located within a small grove of pine trees next to a decorative pond in a condominium development. The tank battery is comprised of one 210-barrel tank and mechanical separator in a chain-link enclosure located 1,000 feet east of the wellhead, and accessed from Jackson Street. Fittings and connections at the battery are secure. Tank lids are closed and secure. The enclosure gate is locked.

The spacing is in accordance with the rule, and the valves and mechanicals are in good repair. The dike is very low throughout. I did not observe any signs of leaking product or gas vapors. I called Tim Reilly of Great Plains and advised him about the need for dike repair. I indicated that the requested date for completion is 12/31/2012.

The wellhead and battery are located in a residential area along Jackson St. in Mentor. The wellhead is located within a partially enclosed green space within a condominium community. The wellhead is comprised of a mechanical plunger lift system and is located within a chain-link enclosure. The enclosure is surrounded by mature evergreen shrubs and pine trees, obscuring it from view. The battery is located to the east, along Jackson Street, and is accessed directly from a short dedicated drive from Jackson St. The battery is comprised of a 210-barrel steel tank and separator unit. Connections and fittings are secure and free from signs of leaks or problems. Signage is present as well as secondary containment. At the time of this inspection, there is no signage at the wellhead which is remotely located from the battery. I contacted Great Plains and requested that they place signage at the wellhead.
