I contacted representatives with Arysta and advised that they contact ODNR Surety & Bonding to resolve the issue. I also advised Bill that M & R should provide whatever production data was missing during the time which Arysta Life Science reported non-production, and the period in which M & R was producing the well. |

Elements of the well consist of a production wellhead with mechanical plunger lift system. The wellhead is located in a chain-link enclosure. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located north of the wellhead. The battery is comprised of 2 steel 100-barrel tanks, and a horizontal separator. There is an ID placard at the wellhead, but its in very poor condition and needs replaced. The wellhead and battery are located in an area currently being used for commercial agriculture. The local topographically is level and there are no visisble water courses or streams near the unit. ase road. Access to the unit is currently afforded via a common utility road for the adjacent overhead power lines, with entry points at Shepard Road and Lane Road. I will contact the well owner about replacing the ID placard. There are otherwise no issues or problems requiring attention at this time. |
| 1530201684 | 2018-06-28 | UrbanProduction Wells | No Violations | I inspected this well as part of routine inspection activity and as a follow-up to previous inspections in which I had noted that M&R Investments Ohio, LLC (M&R) was apparently actively producing the well, although ownership of the well appeared to be listed under Arysta Lifescience North America (Arysta) . At that time, I had contacted Arysta and found that they were unaware that the well was in active production by M&R. I contacted Bill Fitsmartin with M&R and he indicated that the well was an their list of wells actively owned and operated by M&R. I advised Arysta to contact M&R through Bill Fitzmartin to resolve the ownership issue. At the time of this inspection, there had not been any change in the ownership listing. I contacted the listed owner and a response was pending. |

I had previously noted that the well was being actively produced by Diversified Investments. Subsequent discussions with both Diversified Investments reps and Arysta Lifescience North America indicated there was uncertainly about who the actual owner was. Since the time of that discussion, Bill Fitzmartin with Diversified Investments had indicated that he confirmed that Diversified did not own the well and had discontinued production of the well. The well and associated battery are located on a small agricultural parcel. At the time of this inspection, fittings and connections at the wellhead plunger-lift system and the battery were secure. The well was in production with active charter record. Access to the well is afforded by a gravel-paved road (Kenner Rd) that enters from Middle Ridge Rd. The road was dry and accessible. The insurance certificate was current. There had not been reportable production since 2012. The ID signage was not complete or current. I contacted the owner (Arysta Lifescience North America) at the phone number on record and left a message. A response was pending at the time of the time of this report filing. |
