0240769387 2018-08-13 Orphan No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for non-production. It was subsequently determined that the well owner was unresponsive and no longer a viable entity. The well had since been referred for inclusion in the Idle & Orphan Well Program. The well will be dispensed accordingly through that program.
0851225777 2017-02-21 Orphan No Violations I inspected this well as part of an evaluation for possible inclusion into the Idle & Orphan Well Program. At the time of this inspection, I observed the following: The production system was comprised of a wellhead and associated tank battery located within a active commercial nursery operation. The surrounding areas were comprised of growing operations, and associated access roads. The wellhead was comprised of a casing riser with mechanical plunger-lift system and flow-line connections that were closed. There was an associated tank battery comprised of one 210-barrel steel production tank, one 100-barrel steel brine tank (partially buried), and one horizontal separator. The fittings and connections at the wellhead and battery exhibited severe corrosion with no signs of recent maintenance. I noted gas actively leaking from the 3.5-inch casing riser near the surface casing bushing. The flow connection valves at the wellhead were closed. The shared-use, gravel-paved access road was rutted from use but driveable.
1104244766 2012-02-01 UrbanProduction Wells Violations Noted Well has a 210BBL steel production tank and a 50 steel brine tank that is 80% under ground. 50BBL brine tank needs cleaned and removed. Identification sign needs posted. The nursery is using the wells production. There has not been any production reported to the Division of Oil and Gas Resources since 1993. Bond is current on the well, operator needs to submit a current certificate of insurance. Dike needs built up around the battery. Well needs a current identification sign posted. Well needs put into commercial production or production needs reported for the past 19 years. The well being used by the nursery is commercial production.
1452546557 2016-01-05 UrbanProduction Wells No Violations I inspected this well location as part of an evaluation of current site conditions. The well and battery are located within a parcel utilized for commercial agriculture. At the time of this inspection, the following elements were present: a wellhead with visible 8 5/8-inch surface casing, 3.5-inch production casing, and plunger lift system. There was gas leaking from the 3.5-inch casing, near the surface casing bushing. There was a battery with one 210-barrel steel production tank, one 100-barrel steel brine tank that was partially buried, and separator unit. The various elements show significant corrosion and lack of maintenance. Areas beyond the wellhead and battery consist of nursery grounds and associated operations. Access to the wellhead is via shared-use roadways. To date, the owner of record has not responded to attempts to contact them. Liability insurance coverage has lapsed.
1488900428 2017-03-07 Orphan No Violations My inspection found the well to be equipped with 8.625 and 3.5 casing. The well casing is plumbed up above ground and has a wellhead tree in place. The 3.5 casing is rusted through at the 8.625 to 3.5 packing seals. The previous owner has installed a line clamp on the 3.5 casing leak. The well is currently leaking natural gas from around the line clamp. There is a 2 flow line plumbed from the wellhead to the tank battery. The tank battery consists of a 210 BBL production tank, a buried 50 BBL brine tank, and a mechanical separator. There are no overhead obstructions above the tank battery or the well. The well is easily accessed from a previous lease road.
1543608495 2018-11-30 Production Wells Violations Noted At the time of my inspection, I parked my truck east of the well and existed my vehicle, I could hear the well leaking from 200 away. The well is equipped with a plunger lift system. The production casing is three and a half (3 1/2) inch. There is a repair clamp on the three and half inch casing above the wellhead, the production casing is leaking behind the clamp. The well has defective production casing, that requires the casing to be repaired or the well to be plugged. The tank battery is located to the east of the well. The battery consists of a separator unit, two-hundred-and-ten-barrel oil production tank and a buried steel tank. Buckeye Resources Inc. has a status of canceled with the Secretary of State. John M. Ackerman owner of Buckeye Resources Inc. is deceased. David E. Railsback estate attorney for Mr. Ackerman provided the Division with a letter on 12.17.2010 stating that oil and gas Interest for this well were transferred to Kentucky Oil Company. This well is documented as being owned by Buckeye Resources INC. with the State of Ohio. This compliance notice will be sent to the last documented law office of Mr. Railsback and to Kentucky Oil Company at 5112 Portage Road, NW, North Canton, Ohio 44720. The well has defective three-and-a-half-inch production casing and is violation of 1509.12 (A). The wells production casing needs repaired or the well plugged by 12/07/2018. Failure to repair the casing or plug the well by 12/07/2018 will result in an Order to Plug being issued. Non-compliance with the Plug Order will result in Bond Forfeiture.
1544822864 2018-12-14 Production Wells No Violations At the time of my inspection the well remains in non-compliance with Ohio Revised Code 1509.12 (A). The three inch production casing is leaking at the wellhead. The leak can be heard from over one hundred feet away to the east. The three inch production casing is open and the casing pressure is being held back at a ball valve on the pipeline. The ball valve is located next to the separator where the pipeline comes out of the ground and enters the production equipment. The ball valves threaded fittings are leaking (odor and leaking noise where observed.) Well has not been plugged nor has the three inch production casing been repaired. The owner for this well is deceased, the bond for the company is on file. Order to plug the well will be issued.
1552316672 2019-03-11 UrbanProduction Wells No Violations I conducted a compliance notice follow-up for compliance notice 1543608495, regarding repairing the leak on the well casing and/or plugging the well by 12/07/2018. At the time of this inspection, the well casing has not been repaired, nor has the well been plugged. The defective 3 1/2 inch production casing, is required to be repaired or the well plugged per 1509.12 (A).

1787712153 2014-10-23 UrbanProduction Wells No Violations I inspected this producing unit to assess current condition. This well has not had reported production on record. The owner of record is no longer a viable entity and has forfeited bond. Currently there is a 210-barrel production tank and mechanical plunger lift located within a commercial nursery facility. I will recommend this well for inclusion in the Orphan Well Program.
