0096549054 2014-10-23 Orphan No Violations I inspected this well as part of evaluation for inclusion in the Orphan Well Program. The well is comprised of a valve tree with closure swage which is closed and not leaking. The associated tank battery is located adjacent to the Middle Ridge Road. Access to the wellhead is via a short shared use farm road. I met with Rob Hejduk, the current landowner, and he has not current interest in the well foo domestic use.

0603941767 2017-02-21 Orphan No Violations I inspected this well as part of an evaluation for possible inclusion into the Idle & Orphan Well Program. At the time of this inspection, I observed the following: The production system was comprised of a wellhead and associated tank battery located on a parcel being utilized otherwise for agricultural use. The surrounding areas were comprised of level, plowed fields with access afforded by gravel-paved, shared-use roads. The wellhead was comprised of a casing riser with mechanical plunger-lift system and flow-line connections that were closed. Also staged adjacent to the wellhead was a 100-barrel steel tank, and a 500-gallon, horizontal steel tank with a flow-line connection from the production tubing. There was an associated tank battery comprised of two 210-barrel steel production tanks, two horizontal separators. The battery is shared with the Hejduk #2 (#2-0162) located across Middle Ridge Road, on the adjacent property. The fittings and connections at the wellhead and battery exhibited severe corrosion with no signs of recent maintenance. I did not note any signs of leakage of gas or liquids from the well, battery, or associated fittings. The gravel-paved access road was rutted and pot-holed, but was drivable to the wellhead.

The tank battery is equipped with two 210 barrel production tanks and two mechanical separators. There has not been any production reported for this well since 1993. Identification signs need posted at the wellhead and at the tank battery. This well needs put into production or plugged. Temporary inactive status can not be approved for this well.
1025418802 2018-08-13 Orphan No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for non-production. It was subsequently determined that the well owner was unresponsive and no longer a viable entity. The well had since been referred for inclusion in the Idle & Orphan Well Program. The well will be dispensed accordingly through that program.
1097129668 2016-01-05 UrbanProduction Wells No Violations I inspected this well location as part of an evaluation of current site conditions. At the time of this inspection, the well is located in an open portion of a property otherwise utilized for agricultural purposes. At the time of this inspection, the following elements were present: a wellhead with visible 8 5/8-inch surface casing, 3.5-inch production casing, and plunger lift system. Adjacent to the well were two 100-barrel steel tanks. There was a battery located near the access road, comprised of two 210-barrel steel production tanks, and two horizontal separator units. The battery is shared with the Hejduk #2 well, which is located on the south adjacent parcel. The various elements displayed significant corrosion and lack of maintenance. There was a domestic connection at the battery. To date, the owner of record has not responded to attempts to contact them. Liability insurance coverage has lapsed.

1196868668 2007-11-15 Production Wells Violations Noted Received phone call from Cindey Dovkovak 440-773-9812 She lives at 5918 Middle Ridge Road. She states that from Monday, November 12th to today,15th that at approximately 3:00 am every morning that she has been woken up with a loud noise coming from the well area.

I inspected the well and tank battery and found no evidence of any gas bypass or oil spray. The only violation found was that the ID sign is not up to date with emergency contacts. Called Mrs. Ackerman, and advised her that the ID sign needs to be updated . I reinspected the site at 2:30 am and at 3:05 am. The only sound I heard was a train off to the southwest of the house. Case closed for noise at the site.

1489415063 2017-03-13 Orphan No Violations My inspection found the well to be equipped with 3.5 casing. The 8.625 casing is not visible. The well casing is plumbed up above ground and has a wellhead tree in place. There is a 100 BBL tank and a 35 BBL horizontal tank next to the wellhead. There is a 2 flow line plumbed from the wellhead to the tank battery. The tank battery consists of two 210 BBL production tanks and three mechanical separators. The tank battery is being shared with another orphan well permit 2-0162. There are no overhead obstructions above the tank battery or the well. The well is easily accessed from a previous lease road.
