1470420782 2016-08-05 UrbanProduction Wells No Violations I inspected this well location pursuant to a complaint (# 5393) reported from Dominion EOG. According to the complaint, leak detection crews identified gas in subsurface test holes on the south side of the building. During the process of investigating, the crews located a gas well in a subsurface vault with leaking gas observed. Upon my inspection, I noted the following: a nominal 4 x 4 square vault with a wellhead located 2 below grade. The vault was covered with a wooden lid which was, in turn, covered by a metal plate. There was a 1.375-inch horizontal supply line nipple and fitting with valve. The line was connected to a .75-inch copper supply line which exited the vault through a penetration on the west side. There was water which had collected in the bottom of the vault, and I observed some bubbling gas next to the casing. I met with Larry Gregerson, Operations Manager for Cortest, located at 38322 Apollo Parkway. Mr. Gregerson reports that the owner of Cortest, also owns the building next to which the well is located. Upon further inspection, I followed the copper supply line from the well into the building. The supply line was connected to two garage heaters in the adjacent building. I indicated to Mr. Gregerson that I would provide a list of oilfield contractors to assist with identifying the source of the leak and perform repairs as needed. I also indicated that I would provide instructions on the necessary steps to properly register the well as required. Dominion EOG field crews have indicated that they will take no further action at the time of this inspection since the leaking gas was not causing an immediate threat or problem.
2132801586 2017-11-07 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previous inspection in which I had found a wellhead in a shallow vault structure adjacent to a light industrial building. At the time of the previous inspection, Larry Gregerson, Operations Manager for Cortest, had indicated that Cortest was utilizing gas from the well and claimed ownership. As such, I had instructed Mr. Gregerson on the steps needed to register the well. At the time of this inspection, there had not been any apparent attempts by Cortest to complete the registration process. I called and left a message for Mr. Gregerson, and sent an email to same. A response was pending at the time of this report filing.
