Breakdown pressure was 8868 psi, maximum pressure was 11187 psi, average pressure was 10445 psi, maximum rate was 86 barrels per minute (BPM) and average rate was 84 bpm. Initial shut in pressure (ISIP) was 6068 psi and 5 minute ISIP was 5608. Frac gradient was 1.07. 13.375 x 9.625 annular pressure is 200 psi.
0531942725 2019-01-03 Drill / Deepen / Reopen No Violations PAD ADDRESS: 834 Hill Street, Rayland. On the day of my inspection I witnessed the cementing of the 3H. This well will be spudded this afternoon by Precision and they are planning on cementing conductor casing tomorrow if all goes well.
1203550009 2019-06-06 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad contains two wells awaiting drill out operations. Contractors are onsite building production equipment. Well connect construction occuring outside of the pad on the east side. Containment remains from frac. Seven frac tanks, a sand tank, and 2.875 tubing is onsite ahead of service rig. The 13.375x9.625 annular pressure was zero and the 9.625x5.5 annular pressure is 3200 psi. Contacted Andy Ingram with Ascent and informed him of the pressure. He said that he would have it bled down when drill out is on location. Site is stable and vegetated. Pad meets requirements of 1509.072(A) for preliminary restoration.
1546629913 2019-01-04 Casing String Witnessed No Violations On the day of my inspection, I witnessed Precision Rathole had augured a 36 inch diameter hole to 99 feet. 101 feet of 30 inch diameter conductor casing was run. C & J Energy Services was onsite with 290 sacks of class A cement. Cement weighed with mudscale at 15.7 ppg and yield of 1.19. 37 percent excess cement was ran. Cement was displaced with 74 barrels of fresh water. 12 barrels of cement returned to surface.
1547712055 2019-01-17 Casing String Witnessed No Violations On the day of my inspection I witnessed the mine string cement job. Patterson 795 drilled a 24 inch diameter open hole to a depth of 241 feet ground level. Kelly Bushing is 30 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 99 feet ground level. Wyoming Casing ran 274 feet of 18.625 inch diameter casing, j-55, 87.5 lbs/foot, with 41 foot stick up above ground level. Shoe track is 22 feet long. Cement Baskets were set at 154 and 159 feet ground level. Void encountered at 173-189 feet ground level. Schlumberger was onsite with 538 sacks of class A cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 74.5 barrels of fresh water. 25 barrels of gel pumped ahead of cement. No cement returned to surface. 6 barrels of cement pumped down backside to set up on basket. An additional 6 barrels of cement was pumped down backside after a 2 hour wait time. An additional 70.8 barrels of cement pumped. 10 barrels of cement weighing 14.6 ppg return to surface.

1548433586 2019-01-24 Drill / Deepen / Reopen No Violations On the day of my inspection I was accompanied by Inspectors Jeff Adams and Shawn Murphy. We witnessed Patterson 795 drilling a 12.25 inch diameter hole on air at 7179 feet. Estimated total depth of the 12.25 section is 8800. Company man Russ White said that they have not encountered any water or gas while drilling. Company man was instructed to get a vac truck out to suck water out of containment as it was nearly overtopping near the pad entrance.
1548778115 2019-01-29 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 795 tripping into the wellbore. 8.75 inch diameter hole was drilled to 10249 KB. KB is 30 feet. Curve was built from 8864-10188 KB. They are currently tripping in the hole with an 8.5 inch diameter bit. Current mud weight is 12.5 ppg and the have a continuous 1-3 foot flare when drilling below the curve.
1549300951 2019-02-04 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 795 circulating the hole in preperation for the 5.5 inch diameter production casing cement job. Currently there is a 5 foot flare burning. Company man Larry hart said he will have the well static prior to pumping cement. Current mud weight is 13.3 ppg. Patterson 795 drilled an 8.5 inch diameter open hole from 10249-20937 feet. Previous casing set and cemented was 9.625 inch diameter intermediate casing to a depth of 8808 feet. An 8.75 inch diameter hole was drilled from intermediate shoe, through curve to 10249 feet. Wyoming Casing ran 20940 feet of 5.5 inch diameter casing, p-110, 23 lbs/foot, with 3 foot stick up. Casing was centralized. Shoe track is 47 feet long. Schlumberger was onsite with 3847 sacks of class H cement. Cement weight is 15.6 ppg and yield of 1.1. 75 barrels of 14ppg spacer planned to pump ahead of cement. All numbers are from KB. KB is 30 feet.Did not witness job.

Wyoming casing ran 20940 feet of 5.5 inch diameter casing, p-110, 23 lbs./ft, with 3 foot stick up. Schlumberger was onsite with 3847 sacks of class H pozblend cement weighing 15.6 and 1.1 yield. They will pump 75 barrels of 14 ppg spacer ahead of cement. Mud weigh prior to job is 13.3 ppg. Rig is circulating gas out of well. Pump job not wintessed.
