0184076000 2019-01-14 Completion Testing No Violations On the day of my inspection I witnessed a Universal frac crew on location. They are currently tied into the 2H and 4H wells for zipper fracking. This well has not been connected to yet. Surface annular pressure is 0 and intermediate annular pressure is 308.
0333790899 2019-06-05 Production Wells No Violations On the day of my inspection I was accompanied by Supervisor Eric Conrad. We witnessed that the pad is currently in flowback operations. There is no tubing in the well and it is producing out of the 5.5 inch diameter casing. The 13.375x9.625 annular pressure is 318 psi and the 9.625x5.5 annular pressure is 2 psi. Production casing has 5159 psi. Permanent production equipment onsite includes four line heaters, four GPUs, eight sand seperators, two 500 barrel steel tanks, and one combustor. Temporary flowback equipment includes two line heaters and two sand vessels.

-150), 15 gallons of (FE200A) Iron Control ran in the acid. There were 42 gallons of biocide (BIOC11139A), 101 gallons of (CS-1000 SI) Scale control and 145 gallons of friction reducer (FRW-1450) ran. Due to the pressures coming up at the end of the job there was 10 gallons of gel (HVG-1) and Gel Breaker (APB-1) ran. The total of pumps inline were at 19. The open pressure was at 4,601 psi and the break down pressure was at 8,650 psi. The average pressure was 10,309 psi and the average rate was at 76.1 barrels per minute. The displacement or flush was at 318 barrels of water. The initial shut in pressure is at 6,416 psi and 5-minute pressure is at 6,073 psi. The surface annulus pressure was at 2,000 psi and the intermittent annulus pressure was at 450 psi.

0578753554 2019-05-16 Completion Testing No Violations On the day of my inspection I witnessed the drill out crew is no longer on location. The 13.375x9.625 annular pressure is 250 psi and the 9.625x5.5 annular pressure is 3000 psi. Contractors are onsite placing production trees on the wells.
0598388874 2019-04-10 Completion Testing No Violations On the day of my inspection I witnessed CWS stand alone snubbing unit onsite. They are currently drilling out frac plug 36/69. 13.375 x9.625 annular pressure is 350 psi and 9.625x5.5 annular pressure is 900 psi. 2h, 4h, and 6h wells have been drilled out and are awaiting flowback operations. Contractor are onsite installing production equipment.
0731771460 2019-04-22 Completion Testing No Violations On the day of my inspection I witnessed a CWS stand alone snubbing unit onsite. Snubbing unit is on this well. There is an obstruction in the wellbore at 15778 feet. They are running a downhole camera to try and figure out what is stopping them. 13.375x9.625 annular pressure is 250 psi and 9.625x5.5 annular pressure is 1800 psi.
0843166599 2019-02-13 Completion Testing No Violations The inspection revealed the FTS service company is rigging equipment up. Universal well services were on site and there was issue with treating iron and fittings. The frac has currently not started. I talked to William and Homer Bowman the consultants over the completions. They will call when they are up and running again.
1339258789 2019-01-10 Completion Testing No Violations On the day of my inspection I witnessed a Universal frac crew on lcoation. They are tied in to the 4H and the 2H well. They have not tied into this well yet. Could not read surface annular gauge as there was pressure on the lines. Intermediate annular reads 380 from transducer.
1538094358 2018-09-27 Casing String Witnessed No Violations On the day of my inspection I witnessed that Falcon Rig 26 had drilled the 24-inch diameter hole to a depth of 322 feet. Wyoming Casing crew ran 313 feet of 18.625-inch diameter J 55 casing at 87.5 pound per foot, with 3.6 feet of casing above Kelly Bushing, which is 13 feet above ground level. Cement basket was set at 246 feet. C&J Energy Services was on site and rigged up their cement head and loaded lines with 5 barrels of fresh water and pressure tested to 1500 pounds per square inch. C&J Energy Services established circulation with 70 barrels of fresh water and a 35-barrel gel spacer, washed up and then pumped 129 barrels 605 sacks of 15.6 pounds per gallon with a yield of 1.2 cubic feet per sack Class A cement mixed with 5.24 gallons of water per sack. Cement was displaced with 88 barrels of fresh water, with 5 barrels of good cement to surface. Plug was bumped at 100 pounds per square inch and pressured up to 640 pounds per square inch floats held with .5 barrels of flow back. I informed Ascent company man Randy Shepard that the casing must remain static until cement reaches a compressive strength of 500 pounds per square inch.
1557254287 2019-05-07 Completion Testing No Violations On the day of my inspection I witnessed a CWS stand alone snubbing unit onsite. They are curently drilling out frac plugs at 15807 feet. 20 plugs remain. 13.375x9.625 annular pressure is 250 psi and 9.625x5.5 annular pressure is 1800 psi. Company man Gary Seago anticipates moving off of the pad by end of week.
