0040620364 2018-10-30 Completion Testing No Violations On the day of my inspection I witnessed a Universal frac crew on location. During my inspection they were stimulating the 4h well. Stage 74/74 on this well is next for stimulation. Plug is set at 9479, with bottom perforation at 9468 and top perforation at 9327. After stimulating the final stage they will set 2 kill plugs. First kill plug will be at 8574 and the second will be at 8474. Surface annular pressure is 450 and the intermediate annular pressure is 530. Identified myself to company man William Bowman.
0287613259 2018-10-02 Completion Testing No Violations On the day of my inspection I identified myself to Company Man Bub Ruckert. Universal frac crew is beginning to move onto location this date. Currently trucks are bringing water in to fill the above ground storage tank. Surface and intermediate annular pressure gauges read 0 and 650 psi, respectively.
0740233274 2018-07-17 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Xtreme 802 making a bit trip. The 12.25 inch diameter hole is drilled to 7438 feet. Total depth for this intermediate section is planned at 8300. Walk around location revealed no issues with containment.
1091082701 2018-07-12 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Xtreme 802 drilled a 17.5 inch diameter open hole to a depth of 1395 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming casing ran 1391 feet of inch diameter casing, j-55, 61 lbs/foot. Shoe track is 43 feet long. Schlumberger was onsite with 1312 sacks of class A cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 30% excess ran in open hole. Cement displaced with 205 barrels of fresh water. 25 barrels of gel pumped ahead of cement. 50 barrels of cement returned to surface, and floats held with 1 barrel back.

Wireline is running into stage 45/74 to set plug and perforate. Plug will be set at 14487, bottom perforation is at 14396, and top perforation is at 14256. Identified myself to company men Wendall Braddy and William Bowman. Surface annular pressure is 300 and intermediate is 530
1485357172 2018-07-24 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Xtreme 802 drilling an 8.5 inch diameter hole at 13360 feet. Kick off point was 8575 and landing point was 9620. A small flare is burning during this inspection. They are drilling with 12.4 ppg oil based mud. Planned total depth of well is 22400 feet. Containment appears to be functioning as desgined.
1530823827 2018-07-05 Drill / Deepen / Reopen No Violations On the day of my inspection I found Xtreme 802 on location. The derrick is partially standing and company man heath says it will be completely up by this afternoon. Heath thinks they will likely spud on Monday the 9th. The conductor is the only casing set and he asked informed to call me prior to the surface cement job.
1531325511 2018-07-11 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Xtreme 802 drilling a 17.5 inch diameter hole on air. They have drilled 1110 feet and and will reach total depth for this section around noon. Company man John Wilson estimates the 13.375 inch diameter cement job will be around 2 am on 7/12. Walk around location revealed no issues with containment.
1533040388 2018-07-31 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Xtreme 802 had reached total depth of 22422 and Schlumberger had cemented the 5.5 inch diameter production casing last night. Rig is preparing to skid to the 4H well. It appears that there is some hydraulic oil inside of the containment beneath the catwalk. Company man Heath was notified to get this cleaned up.

1652040793 2019-04-25 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad to contain two properly labeled, producing wells. This well is producing through the 5.5 inch diameter casing. This well is producing from each side of the well head. Production equipment includes four sand seperators, two GPUs, two line heaters, gas buster, and two 500 barrel steel tanks, in their own containment. Six FTS trucks remain on location. The 13.375x9.625 annular pressure is 263 psi and the 9.625x5.5 annular pressure is 650 psi. Pad slopes are stable and vegetated except for pipeline right of way which is no fully vegetated. Site meets requirements of 1509.072 (A) for preliminary restoration.
1718081423 2018-11-05 Completion Testing No Violations On the day of my inspection I found this pad to contain two wells that have been stimulated and not drilled out. Identified myself to company man Josh McTheny. He said that a snubbing unit will be moving onto location later today for drill out. Surface annular pressure is 450 and intermediate is 700.
