0057137130 2019-01-22 Completion Testing No Violations On the day of my inspection I witnessed FTS on location and rigged up to both these wells. They are currently swapping out blenders and are not pumping on the wells. Wireline set stage 13 plug at 21048, with bottom perfs at 21036 and top perfs at 20896. Surface annular pressure is 0 and intermediate annular pressure is 175.

1536678076 2019-02-13 Completion Testing No Violations The day of inspection revealed that FTS service company was on site and had fraced 74 out 77 stages. There were 230,000 lbs. of proppant ran, with 40,000 lbs. of 100 mesh sand ran and 190,000 lbs. of 30-70 mesh ran. The water content ran was at 4600 barrels, ran 2000 gallon of 7.5 % of Hydrochloric acid, CI 150-2 gallon of acid inhibiter, 10 gallon of iron control, 100 gallon of friction reducer, 40 gallons of biocel, 40 gallon of clay seal, no gel ran. The break down was 8950 psi. The intermittent casing annulus pressure was at 250 psi and the surface casing annulus pressure was at 1500 psi. The wind was approximately at 40 mph and the recommended wind should be below 20 mph. The tool on the frac head is approximately 50 feet in length. The frac job will be placed on hold till the wind calms down.
1537884967 2018-09-25 Casing String Witnessed No Violations On the day of my inspection I witnessed the mine string cement job. Patterson 584 drilled a 24 inch diameter open hole to a depth of 276 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming casing ran 280 feet of 18.625 inch diameter casing, j-55, 87.5 lbs/foot with 4 foot stick up. Shoe track is 44.6 feet long. Cement Baskets were set at 215 feet. Void encountered at 224-238 feet. Schlumberger was onsite with 609 sacks of class A cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 61 barrels of fresh water. 25 barrels of gel pumped ahead of cement. No cement returned to surface. 5 barrels of cement pumped down backside to set up on basket. Rig ran 200 feet of 1 inch diameter tubing. An additional 15 barrels of cement was pumped down backside after a 2 hour wait time. An additional 5 barrels pumped after 2.5 hour wait time. Rig ordered more cement. An additional 15 barrels of cement was pumped after 2 hours. Finally the backside was topped out with 40 barrels of cement. 8.5 barrels returned to surface weighed 15.4 ppg.
1538054657 2018-09-27 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job with Paterson Rig 584 on location and rigged up on the hole. The contractors are Schlumberger Cement Service and Wyoming Casing. The rig has drilled a 17.5 diameter well bore to a measured depth of 1,585 with the depth measured off a drilling rig Kelly Bushing heigth of 25 above ground level elevation. Upon reaching total borehole depth the rig pulled the drill string and inserted 1,576 of 13.375 diameter J-55 grade buttress thread casing with a casing float shoe and collar casing equipment ran along with centralizers placed on every 3rd casing collar. The rig circulated the last few casing joints into the well bore and established circulation at that point. Schlumberger Cement Service cemented the casing by first bumping 25 barrels of 8.6 pound per gallon gel spacer followed by a 273 barrel cement slurry mix consisting of 1,275 sacks of class A cement. The cement weight was verified at 15.6 pound per gallon by weight of scale and had a yield of 1.2. The cement was displaced with 230 barrels of fresh water with 83 barrels of cement returns witnessed to surface. The plug was landed and casing pressure tested to 1,087 pounds pressure.
1538073977 2018-09-26 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 584 perform a 30 minute hydrostatic test. The rig drilled a 17.5 inch diameter hole to 282 feet. 18.625 inch diameter casing shoe is at 276 feet. Water remained in wellbore for 30 minutes and did not fall.
1538658697 2018-10-04 Casing String Witnessed No Violations On the day of my inspection I witnessed the second stage of the 9.625 inch diameter intermediate cement job. Patterson 584 drilled a 12.25 inch diameter hole to 8727 feet. Wyoming casing set casing at 8721 feet. Casing is L-80, 40#/ft. Shoe track is 91 feet long. DV tool is at 4255. For the first stage Schlumberger had 146 barrels of lead cement with a weight of 13.5 ppg and yield of 1.67. Tail cement was 186 barrels of 15.6 ppg and a yield of 1.19. 40 barrels of 13 ppg mud push was pumped ahead of lead cement. First stage displaced with 654 barrels of 12# oil base mud. Rig circulated on well after DV tool opened. No cement to surface and 20 barrels of spacer for first stage. Second stage pumped 284 barrels of cement weighing 15.6 ppg and 1.19 yield. Displacement was 322 barrels of displacement. 43 barrels of cement to surface and 40 barrels of spacer. Identified myself to Company man Tommy Bell.
1539634266 2018-10-15 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 584 drilling an 8.5 inch diameter open hole at 12200 feet. Identified myself to Company man Tommy Bell. A few small tears in the containment were noted and vendor was on location assessing the damage for repair. There was a small flare breifly during my inspection.
1591317053 2018-09-13 Casing String Witnessed No Violations On the day of my inspection I witnessed Precision had augured a 36 inch diameter hole to 99.5 feet. 102 feet of 30 inch diameter conductor casing was run. Petroset was onsite with 315 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 50 percent excess ran. Mix water tested 72 degrees Fahrenheit, pH of 7 and 36 ppm chlorides. Cement was displaced with 74 barrels of fresh water. 16 barrels of cement returned to surface.
1716574192 2018-10-23 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 584 on location. They reached total depth of 23195 this morning with an 8.5 inch diameter hole. Mud weight is up to 15.2 ppg. Company man Josh Delso informed me that they had a spill yesterday when a hose failed and released an estimated 20 barrels of oil based mud onto containment. 4 pressure washing crews were on location cleaning the rig during my inspection. No indication of oil base mud outside of rig containment was noted. Rig is circulating on wellbore and has not started to trip pipe.
