Stages 1-26 have been stimulated on this well. Stage 26 was completed today. Plug was set at 21649, with bottom perforation at 21638 and top perforation at 21497. 3000 gallons of 7.5% HCl pumped along with 5192 barrels of slurry. 0012032874 2018-08-29 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 3 unlabeled wellheads. These wells have not been stimulated. Weld inspector is onsite x-raying the pipeline to the pad. Surface and intermediate gauges are in cellar, underwater, and unable to be read. 5 frac tanks are on location and no other equipment. Pad is stable and vegetated. Site meets requirements of OAC 1509.072A for preliminary restoration.
0391277587 2018-02-16 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 1670 feet. Previous casing set and cemented was 18.625 inch diameter mine string casing to a depth of 322 feet. Wyoming Casing ran 1663 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot with a 3 foot stick up. Casing was centralized with 14 centralizers. Shoe track is 44 feet long. CJ/O-tex was onsite with 1315 sacks of class A cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 40% excess ran in open hole. Cement displaced with 246 barrels of fresh water. 35 barrels of gel pumped ahead of cement. 49 barrels of cement returned to surface, and floats held with 1 barrels back.

This well section has a 6 degree tangent. Ascent Superintentant Stan Cherry and Company Man Van Dietrich provided a rig tour for ODNR supervisors Brent Bear and Mike Moreland, myself, and others from our legal section and Attorney Generals office.

Currently the 2H well is the only well being stimulated. Company man said that is because of proximatey with drilling rig. Surface annular pressure is 0 and intermediate annular pressure is 929. Company man said are going to bleed pressure off on the intermediate while wireline is running into the 2H.

Plug was set at 21649, with bottom perforation at 21638 and top perforation at 21497. 3000 gallons of 7.5% HCl pumped along with 5192 barrels of slurry. Max pressure was 11393. Average pressure was 10701. Initial shut in pressure (ISIP) was 6906 and 5 minute ISIP was 6605. Surface annular pressure was 0 and intermediate annular pressure was 430.

1217208341 2018-12-06 Completion Testing No Violations On the day of my inspection I witnessed a frac tech crew on location. Identifed myself to company man Doug Wilson. Wireline just finished perforating stage 59/92 on this well during my inspection. Plug was set at 15907, with bottom perforation at 15895 and top perforation and 15755. Surface annular pressure was 100 and intermediate annular pressure was 0.
1516818064 2018-01-23 Drill / Deepen / Reopen No Violations On the day of my inspection I found pad having two auger-type conductor rigs drilling. Company representative Stuart Schwarts said that this well had a 36 inch diameter hole drilled to 30 feet. It was then grouted to surface. It was then drilled with 42 inch diameter bit to a depth of 55 feet. Fiberglass tubing was ran and well was cemented back to surface. No drilling on this well today.
1518629392 2018-02-14 Casing String Witnessed No Violations On the day of my inspection I witnessed the mine string cement job. Falcon 26 drilled a 24 inch diameter open hole to a depth of 338 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming Casing ran 325.6 feet of 18.625 inch diameter casing, j-55, 87.5 lbs/foot, with 3.6 foot stick up. Shoe track is 24 feet long. Cement Baskets were set at 236 feet. #8a coal encountered from 260-266 and mine void encountered from 282-288 feet. CJ/Otex was onsite with 605 sacks of class A cement. Additives include 3% CaCl and 0.2% X-air. Cement weighed with mudscale at 15.8 ppg and yield of 1.17. 50% excess ran in open hole. Cement displaced with 92.5 barrels of fresh water. 35 barrels of gel pumped ahead of cement. No cement returned to surface. 10 barrels of cement pumped down backside to set up on basket. An additional 38 barrels of cement was pumped down backside after a 2 hour wait time. After 2 more hours, 60 barrels were pumped and 26 barrels were returned to surface.

1894681468 2018-07-03 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Xtreme 803 had reached total depth of 26130 feet and are currently circulating the wellbore. Company man Van Dietrich said they plan on tripping out of wellbore in the early morning hours and should be running 5.5 inch diameter production casing Thursday morning. Rig will move off of location after cement job is completed.
1894758428 2018-06-29 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Xtreme 803 drilling their lateral section with an 8.5 inch diameter bit at 10745 feet. Kick off point for curve was 9135 and landed at 10224. Drilling with 13.2 ppg oil based mud. No flare during inspection. Planned total depth is 26,153 feet. Location has standing water over shut pad drains and company man Van Dietrich said construction is supposed to come out and test water to decide to discharge or haul off.
1910943704 2018-11-28 Completion Testing No Violations On the day of my inspection I witnessed FTS on location. I was accompanied by inspector Jeff Adams. Identified myself to company man Jeff Whitehair. They just finished stimulating stage 40/92. Plug was set at 19213, with bottom perforation at 19201 and top perforation at 19061. FTS pumped 5256 barrels of clean water and 2000 gallons of 7.5% HCl. 40080 lbs of 100 mesh and 189989 lbs of 30/70 mesh for a total of 230069 lbs of sand. Break down pressure was 9275 at 25 barrels per minute (BPM). Average rate was 78 bpm with a max rate of 80 bpm. Average pressure was 10742, with a max pressure of 11435. Initial shut in pressure (ISIP) was 6893 with a 5 minute ISIP of 6319. Surface annular pressure was 0 and intermediate annular pressure was 300.
