0058625572 2018-08-29 Production Wells No Violations On the day of my inspection I identified myslef to Construction Consultant Denny Bird. This pad has 3 wells on it. Activity onsite includes building of flowlines and setting production equipment. Wells do not have gauges on surface or intermediate annuli at this time. They are working on bring the above grade. Production equipment includes two 500 barrel steel tanks in their own containment, three GPUs,three line heaters and six sand traps. Wells do not have tubing and will produce through production casing. Flowlines are plumbed from both sides on the wellheads.
0147810077 2018-08-01 Completion Testing No Violations On the day of my inspection I was accompanied by DOGRM inspector Bob Roush. ID to company men Bub Rucker and Brady. Universal frac crew is onsite. Intermediate annular pressure is 470 psi. Wireline is running in the well to perforation stage 50/66. Plug will be set at 12989, bottom perforation at 12978, and top perforation at 12837. Perforated with 5 guns, 4 shots, on 35 foot spacing.
0234648757 2018-06-28 Completion Testing No Violations On the day of my inspection I witnessed Universal frac crew onsite preparing to begin stimulations on this pad. This well has 66 stages. The stimulation should start today with the 1H well. Intermediate annular pressure is 600 psi.
0698621581 2018-01-04 Casing String Witnessed No Violations Pad address is 640 TOWNSHIP ROAD 107A DILLONVALE 43917. On the day of my inspection I witnessed Express had augured a 36 inch diameter hole to a depth of 100 feet. 100 feet of 30 inch diameter conductor casing was run. Express was onsite with 275 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 50% percent excess ran. Cement was displaced with 74 barrels of fresh water. 12 barrels of cement returned to surface.
0719082915 2018-01-23 Casing String Witnessed No Violations On the day of my inspection I witnessed the mine string cement job. Falcon 26 drilled a 24 inch diameter open hole to a depth of 324 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming Casing ran 316 feet of 18.625 inch diameter casing, j-55, 87.5 lbs/foot with 2 foot stick up. Shoe track is 24feet long. Cement Baskets were set at 245 feet. Void encountered at 264-273 feet. O-Tex was onsite with 630 sacks of class H cement. 70% excess ran in open hole. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. Cement displaced with 89 barrels of fresh water. 35 barrels of gel pumped ahead of cement. No returns throughtout job. Bumped plug and held with .5 barrel back to truck. 20 barrels of cement pumped down backside to set up on basket after 1.5 hours. An additional 30 barrels of cement was pumped down backside after a 1.5 hour wait time. Another 34 barrels pumped after 2 hour wait time. Cement tagged at 66 feet down. O-tex ran out of cement and ordered another truck. 42 barrels pumped to top off job 7 hours later.
0729517780 2018-04-24 Drill / Deepen / Reopen No Violations On the day of my inspection I wintessed Patterson 584 was drilling an 8.5 inch diameter hole at 16838 feet. Company man Tommy Bell believes they will setting 5.5 inch diameter production casing on Saturday. There is a lazy flare burning during my inspection.
0871099434 2018-08-06 Completion Testing No Violations On the day of my inspection I witnessed Universal Frac crew onsite. Stimulation has been completed on this pad. Wireline was coming out of the well after setting a kill plug. First kill plug in this well is at 9676. Second kill plug has not been set but company man Brady estimates it will be approximately 150 feet above the first plug.
1069320092 2018-01-24 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Falcon 26 had drilled out below the 18.625 inch diameter mine string shoe. New hole drilled from 324 feet down to 350 feet. Casing shoe at 314 feet. During my inspection the casing was already filled with fluid and I watched it for :30 minutes. No fluid loss noted. Rig was advised to procede.
1223961788 2018-07-18 Completion Testing No Violations On the day of my inspection I found Universal frac crew on location. This well is on stage 3 out of 63. Plug is at 21214, bottom perforation at 21238, and top perforation is at 21062. Surface annular pressure is 0 and intermediate is 650. Currently fracking the 3H well.
1245104066 2018-08-15 Completion Testing No Violations On the day of my inspection I witnessed Deep Well rig 11 was tripping out of the well. ID to company man James Edwards. All 66 frac plugs have been drilled out. Surface annular pressure was 500 psi and intermediate annular pressure was 600 psi. I did not view the gauges as they were tripping pipe. James said these were the numbers from this morning. He also said that the manifold of the production casing had 3600 psi.
1358676847 2018-08-22 Completion Testing No Violations On the day of my inspection I witnessed Stone 15 rig on location. ID to company man James Edwards. All of the frac plugs have been driled out of this well and they are tripping out of the well. At this time to 5.5 inch diameter casing has 3500 psi on it. This well will be produced through the casing and they did not run tubing. Surface annular pressure is 0 psi and intermediate annular pressure is 600 psi.
1410526995 2018-04-16 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 584 drilling at 6486 feet. 13.375 inch diameter surface casing was previously set by tophole rig at 1635 feet. 9.625 inch diameter casing job will be later in week.

Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 1645 feet. Previous casing set and cemented was 18.625 inch diameter mine string casing to a depth of 314 feet. Wyoming Casing ran 1635 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot. Casing was centralized with 14 centralizers. Shoe track is 44.25feet long. O-tex was onsite with 1370 sacks of class H cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 242 barrels of fresh water. 35 barrels of gel pumped ahead of cement. 50 barrels of cement returned to surface, and floats held with 0.5 barrels back.

1998479467 2018-07-03 Completion Testing No Violations On the day of my inspection I found a universal frac crew rigged up to this well, as well as permits 20748 and 20749. Stage one has been completed on this well. Surface annular pressure is 0 and intermediate annular pressure is 753 psi.
