0054391707 2018-05-22 Completion Testing No Violations On the day of my inspection I found a Halliburton frack crew rigged up on the 1h, 2h, and 3h wells. They are currently on stage 15 of 76 on this well, but currently fracking the 1h. 9.625 inch diameter annular pressure is 720 psi. Backside pressures on the 2h is 767 psi and pressure on the 1h is 340 psi. Walk around location revealed no spills. Containment is functioning and pad drains are closed.
0451719878 2018-06-15 Completion Testing No Violations On the day of my inspection I witnessed a halliburton frac crew was onsite. This well has 34 stages of the frac completed. Wireline crew is currently retrieving a bridge plug. Surface annular pressure is 500 psi and intermediate pressure is 484 psi.
0627614225 2018-07-26 Completion Testing No Violations On the day of my inspection I witnessed Deep Well Snub 11 onsite. Rig is drilling out on the 15/50 plug. This plug is at 18182 feet. Surface annular pressure is 750 and intermediate annular pressure is 700. Gauge from last week was on well when I arrived reading 1500 psi from surface annular and David Norris told me that he took gauge of and it zerod out at 600 psi. I had him replace the gauge and replacment gauge reads 750.
0690516508 2017-10-31 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 111 feet and ran 111 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex Pumping LLC pumped 390 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 1.5 percent Calcium Chloride and 0.25 pound per sack cello-flake. O-Tex then displaced with 80 barrels of water, leaving 13 feet of cement in the casing. Good cement returned to surface and remained static. Approximately 18 barrels of cement returned to surface, Ascent left approximately 18 inches of cement in the cellar as a seal.
0827528047 2018-01-30 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 795 was tripping back into the wellbore with a stiff assembly. 8.5 inch diameter hole is currently drilled to 10043 feet. The rig tripped out of wellbore to change from a flexible assembly to a stiff assembly. Curve was built from 8825 to 9840 feet. Containment is functioning as designed.

One stage remains on pad but equipment failure has pad idle. Surface annular pressure is 575 and intermediate annular pressure is 500. Frac stack has been removed from wellhead.

Surface annular pressure is 1350 and intermediate annular pressure is 600. Company Man was notified of pressure of surface casing. He said they would hook up to it and bleed it off.
1645652535 2018-08-01 Completion Testing No Violations On the day of my inspection I was accompanied by DOGRM inspector Bob Roush. ID to company man David Rapattoni. Deep Well Snub 11 rig onsite and set up on the 3H well. Surface annular pressure is 0 and intermediate annular pressure is 600 psi. Drill out is completed on this well.
1649845926 2018-05-16 Completion Testing No Violations On the day of my inspection a Halliburton frac crew is rigged up to the 1H, 2H, and 3H wells. 24 out of 49 stages have been fracked on this well. Frac van had to be moved to run coiled tubing in the 1H. Company men said surface anular pressure was 0 psi and intermediate was between 400-500 psi. I did not walk over to check gauges for safety reasons.
1841533037 2018-06-21 Completion Testing No Violations On the day of my inspection I found a Halliburton frac crew onsite. Company man Gilbert and Paul gave ODNR supervisors Brent Bear and Mike Moreland, myself, and others from our legal section and Attorney Generals office, an informative tour of the location and equipment. They are currently waiting on coiled tubing to fish for lost tools and a retrievable bridge plug in stage 35.

2023191409 2018-10-26 Preliminary Restoration No Violations On the day of my inspection I witnessed this contain contains five properly labeled producting wells. Wells do not have tubing in them and are produced on natural reservoir pressure through the 5.5 inch diameter casing. All wells on the pad are producing from each side of the wellhead. Production equipment includes 10 GPUs, 10 sand seperators, 2 500 barrel steel tanks in their own containment, and 1 combustor. Surface annular pressure is 467 and intermediate annular pressure is 465. Site meets requirements of OAC 1509.072A for preliminary restoration.
2059194601 2018-06-28 Completion Testing No Violations On the day of my inspection Halliburton frac crew had just started stage 49 on this well and the blender broke down. They have pumped a total of 865 lbs of sand prior to break down. Stage will resume when blender is repaired. Intermediate annular pressure is 594 psi.
