0035615811 2017-10-31 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 111 feet and ran 111 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex Pumping LLC pumped 390 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 1.5 percent Calcium Chloride and 0.25 pound per sack cello-flake. O-Tex then displaced with 80 barrels of water, leaving 13 feet of cement in the casing. Good cement returned to surface and remained static. Approximately 20 barrels of cement returned to surface, Ascent left approximately 18 inches of cement in the cellar as a seal.
0773490009 2018-07-26 Completion Testing No Violations On the day of my inspection I witnessed Deep Well Snub 11 onsite. Rig is drilling out on the 2H well. This well has been drilled out. Surface annular pressure is 450 and intermediate annular pressure is 650.
1152485951 2017-12-12 Drill / Deepen / Reopen No Violations On the date of my inspection I found a well maintained lease road with an unlocked gate. The pad is surrounded by an earthen dike with healthy vegetation and a fence with an unlocked gate. Ascent is currently rigging up Patterson Drilling rig 795 on this well. The company man expects that they should be ready to spud on 12/16/17.
1300810154 2018-04-17 Completion Testing No Violations On the day of my inspection I found Weir and Cameron on location putting on the tubing heads and valves respectively. The pad has not been stimulated and they are preparing for Halliburton frack crew.
1325367512 2018-06-15 Completion Testing No Violations On the day of my inspection I witnessed a halliburton frac crew was onsite. Surface annular pressure is 450 psi and intermediate pressure is 550 psi. This well is not rigged up for fracking yet. Currently rigged up on 1h, 2h, and 3h wells.
1514382599 2018-01-03 Drill / Deepen / Reopen No Violations On the day of my inspection I found Patterson 795 drilling the 8.5 inch diameter hole section at 10,139 feet. Company man Russ White stated that the wellbore is not making any fluid and there is currently no flare burning. Containment is functioning as designed. Air temperature is 4 degrees and any water on ground is frozen. Pad drains are shut. 9.625 inch diameter casing cemented on 12-25.
1514904876 2018-01-02 Casing String Witnessed No Violations On the day of my inspection I witnessed Patterson 795 had a drilled an 8.5 inch diameter hole to a depth of 18530. Wyoming casing ran 18535 feet of 5.5 inch diameter production casing, j-55, #/ft with a 5 foot stick up. Halliburton onsite with 3265 sacks of lead cement, with a yield of 1.09 and weight of 15.0. 10% excess ran. Bumped plug, floats held, no cement to surface.
1525757704 2018-07-12 Completion Testing No Violations On the day of my inspection I witnessed Halliburton was rigged up to this well. They started the last stage, 47/47 but the agiator is bent and they cannot pump sand. Top perforation is at 10193 and bottom perforation is at 10334. Perforated with 5 guns having 4 shots each on 35 foot spacing. Surface annular pressure is 475 and intermediate annular pressure is 565. Well is currently shut in until they get agitator fixed and can resume frac. Breakdown pressure was 9305 at 30 barrels per minute.
1678173955 2018-06-29 Completion Testing No Violations On the day of my inspection wireline was coming out of the well after shooting perforations for stage 4. Frac plug is at 17913 feet. Halliburton is onsite but blender broke down and they are waiting on repairs. Intermediate annular pressure is 711 psi.
1809293653 2018-10-26 Preliminary Restoration No Violations On the day of my inspection I witnessed this contain contains five properly labeled producting wells. Wells do not have tubing in them and are produced on natural reservoir pressure through the 5.5 inch diameter casing. All wells on the pad are producing from each side of the wellhead. Production equipment includes 10 GPUs, 10 sand seperators, 2 500 barrel steel tanks in their own containment, and 1 combustor. Surface annular pressure is 24 and intermediate annular pressure is 366. Site meets requirements of OAC 1509.072A for preliminary restoration.

Deep Well Snub 11 rig onsite and set up on the 3H well. Surface annular pressure is 400 and intermediate annular pressure is 650 psi. Drill out has been completed on this well.
